婴幼儿颅外胚胎性恶性肿瘤INI1表达及意义

摘　　要：目的研究整合酶相互作用分子1(INI1)在婴幼儿颅外胚胎性恶性肿瘤中的表达情况,探讨其作为颅外胚胎性恶性肿瘤诊断及鉴别诊断的依据和预后评估。方法应用免疫组织化学方法检测INI1在143例婴幼儿颅外胚胎性恶性肿瘤中的表达。结果9例恶性横纹肌样瘤,肿瘤细胞INI1均表达缺失,缺失率100%,术后化疗5年生存率11%(1例Ⅰ期患儿存活),其余134例肿瘤包括原始神经外胚层瘤、胚胎性横纹肌肉瘤、神经母细胞瘤、淋巴母细胞淋巴瘤、肺母细胞瘤、肝母细胞瘤、肾母细胞瘤等肿瘤细胞INI1均阳性表达,阳性表达率100%,术后化疗5年生存率41%~85%;恶性横纹肌样瘤细胞与其它胚胎性肿瘤细胞INI1标记阳性率比较(P=0.00),差异有显著性;恶性横纹肌样瘤患儿与其它胚胎性肿瘤患儿的5年存活率比较(t≥24.5,P=0.00),差异有显著性。结论应用免疫组化法检测胚胎性肿瘤中INI1的表达是一种方便、可靠的方法,,可用于颅外恶性横纹肌样瘤之诊断及与其它胚胎性恶性肿瘤之鉴别诊断。Objective To study the expression of INI1 in Children extracranial tumors, which might be beneficial for the diagnosis, differential diagnosis and prognostic estimation in the tumours of the extracranial. Methods The expression of INI1 was detected in 143 cases of pediatric tumours in the extracranial with immunohistochemical method. Results Loss of INI1 expression was observed in the 9 cases of MRT, the loss rate was 100%, positive nuclear staining was found in vascular endothelial cell of MRT, the estimated five years survival rate was 11%; but positive nuclear staining of INI1 were found in the 134 cases, including Neuroblastoma, Rhabdomyosarcoma, Pneumoblastoma, Hepatoblastoma,Lymphoblastoma, Nephroblastoma, primitive neuroectodermal tumor, the positive rate was 100%, the estimated five years survival rate were 41%~85%,there was significant statistical differences（t≥24. 5,P=0.00） of five years survival rate between groups. Conclusions Immunohistochemical staining with INI1 is useful in the diagnosis and differential diagnosis of MRT, loss of INI1 expression is a poor prognostic factor. We suggest that INI1 analysis should be performed in all pediatric tumours of the extracranial, especially in embryonal tumours.
