通窍活血汤联合奥拉西坦治疗血管性痴呆的临床分析

摘　　要：目的:研究通窍活血汤联合奥拉西坦治疗血管性痴呆的临床疗效。方法:受试对象来自我院神经内科,共152位。将按随机数字表法分成2组,使用不同治疗方案,对比两组效果,即一组采用奥拉西坦治疗3个月(对照组),一组通窍活血汤联合奥拉西坦治疗3个月(观察组)。结果:观察组总有效率达92.11%,明显高于对照组75.00%。治疗3个月,两组MMSE和HDS均明显升高,ALD明显下降,观察组改善程度高于对照组,两组血液流变学均明显下降,观察组下降程度高于对照组。脑血流动力学平均血流速度均明显升高,观察组升高程度大于对照组。结论:奥拉西坦治疗血管性痴呆基础上,联合通窍活血汤方案对治疗疗效更佳,对血液流变学、血流动力学等均有一定改善作用。Objective： To analyze the clinical effect of Tongqiaohuoxue decoction combined with oxiracetam in the treatment of vascular dementia.Methods： Subjects were selected from March 2014 to January 2017,from the Department of Neurology,a total of 152 cases,were divided into two groups according to the random number table method. Patients in one group were treated with oxiracetam for 3 months（ the control group） and those in the other group were treated with Tongqiaohuoxue decoction combined with oxiracetam（ the observation group）. Effects in two groups were compared. Results： The total effective rate was 92. 11% in the observation group,which was significantly higher than that in the control group with 75. 00%（ P〈0. 05）. For 3 months＇ treatment,MMSE and HDS scores in both groups were significantly higher than those before the treatment（ all P〈0. 05） and ALD scores in both groups were significantly lower than those before the treatment（ all P〈0.05）. Hemorheological indexes in two groups obviously decreased（ all P〈0. 05） and the decreasing level in observation group was higher than that in the control group（ all P〈0. 05）. The mean hemodynamic velocity of cerebral blood flow was significantly increased in both groups and the degree of increase in the observation group was higher than that in the control group（ all P〈0. 05）. Conclusion： Tongqiaohuoxue decoction combined with oxiracetam is more effective on vascular dementia than oxiracetam alone,and can improve hemodynamics,hemodynamics and so on.
