105例白血病患者肺弥散功能及影响因素的临床研究

摘　　要：目的观察分析白血病患者肺弥散功能变化，探讨影响肺弥散功能的影响因素。方法回顾分析我院2012年1月至2017年5月收治的白血病患者105例，应用SPSS19统计软件分析血红蛋白（Hb）校正对肺弥散功能评价的影响。结果以Hb正常、轻度降低、中重度降低进行分组，3组患者校正前DLCO值、单位肺泡容积的弥散量（DLCO／VA）值与校正后DLcOadj值、DLCO／VAadj值比较差异有统计学意义（P〈0．05）；Hb轻度降低、中重度降低组弥散功能分级校正前后差异有统计学意义（P〈0．01）；但Hb正常组弥散功能分级校正前后差异无统计学意义（P〉0．05）；移植、贫血是影响校正前弥散功能分级的高危因素。结论白血病患者DLCO值校正前后有差异，当白血病患者Hb降低时，弥散功能分级校正前后有显著差异，因此在分析白血病患者弥散功能时需行校正，排除Hb的影响。Objective To observe the changes of lung diffusing capacity of leukemia patients, and explore the influencing factors of diffusing capacity. Methods Clinical data of 105 patients with leukemia from January 2012 to May 2017 were reviewed. Diffusing capacity without Hb adjustment and diffusing capacity with Hb adjustment were recorded, and the date were analyzed by SPSS19 software. Results The values of DLCO、DLCO/VA reveal significant changes between Hh uncorrected and corrected in three groups （Hb normal, mild and moderate-severe reduction group）（ P 〈0.05）. The categories of diffusing capacity between Hb uncorrected and corrected group had no difference （ P 〉 0.05）, but there were significant differences in the Hb mild reduction group （ P 〈 0.01） and Hb moderate-severe reduction group （ P 〈 0.01）. Transplantation and anemia were high risk factors that affect the categories of diffusing capacity without adjustment. Conclusions There exist differences in the values of DLCO without/with Hb adjustment of leukemia patients. The categories of diffusing capacity are changed significantly between Hb uncorrected and corrected group when patients with anemia. While clinically analyzing diffusing capacity, the Hb factor should be excluded firstly.
