脑卒中高风险人群高血压情况分析

摘　　要：目的探讨脑卒中高风险人群高血压情况。方法调查382例脑卒中高风险人群的高血压情况,并且进行24 h动态血压监测。结果本组患者中336例(88.0%)患者有高血压,其中212例(63.1%)患者不知晓自身患高血压疾病,226例(67.3%)患者未进行规律的药物控制,19例(5.7%)患者接受高血压药物治疗但未规律检测血压。杓型高血压患者比率最高(222例,66.1%),其次为非杓型(48例,14.3%)、反杓型(43例,12.8%)及超杓型(23例,6.8%)。非杓型及杓型高血压患者高血压知晓率及服药率均较高,超杓型及反杓型高血压患者知晓率和服药率均较低。在合并高血压的脑卒中高危人群中,血压异常节律组脑卒中的患病率(11.4%)显著高于血压正常节律组(6.8%)(P<0.05)。结论脑卒中高危人群中高血压的知晓率、服药率低,且血压异常节律与脑卒中的发病密切相关。Objective To investigate the condition of hypertension in stroke high risk patients. Methods The condition of hypertension was investigated on 382 stroke high risk patients, and patients were received 24 hours ambulatory blood pressure monitoring. Results In this group, 336 cases （88.0%） had hypertension, 212 cases （63.1%） did not know their own hypertension diseases, 226 cases （67.3%） did not have regular drug control, 19 eases （5.7%） received hypertension medication, but the blood pressure was not detected regularly. The rate of dipper type （222 cases, 66.1% ） was the hightest, then there were non dipper type （48 cases, 14.3% ） , reverse dipper （43 cases, 14.8% ） and extreme dipper （23 cases, 6.8% ）. The awareness rate of hypertension and the rate of taking medicine of dipper type and non dipper were high, the awareness rate of hypertension and the rate of taking medicine of reverse dipper and extreme dipper were low. In the stroke high risk patients with hypertension, the prevalence of stroke in the abnormal rhythm group （ 11.4% ） was significantly higher than that in the normal rhythm group （6.8%） （ P 〈 0. 05 ）. Conclusion The awareness rate of hypertension and the rate of taking medicine are low in the high risk population of stroke who are associated with hypertension＇s risk factors, and the abnormal rhythm of blood pressure is closely related to the incidence of stroke.
