转移性乳腺癌患者原发灶PARP-1的表达及与化疗疗效、预后的关系

摘　　要：[目的]探讨PARP-1的表达与转移性乳腺癌患者预后的关系及PARP-1对含铂类方案化疗敏感性的影响。[方法]收集转移性乳腺癌患者的临床病理资料和肿瘤组织蜡块,免疫组化方法检测其术前PARP-1的表达水平。分析原发灶PARP-1表达水平与转移性乳腺癌患者预后及含铂方案化疗疗效的关系。[结果]转移性乳腺癌患者原发灶PARP-1表达阳性率为73.20%,PARP-1高表达患者较低表达患者内脏转移部位更广泛（P=0.025）、CXCR4表达水平高（P=0.003）。PARP-1高表达者较低表达者含铂化疗敏感性差（P=0.000）。PARP-1高表达的患者无疾病生存期（DFS）和总生存期（OS）较低表达者短,但差异均无统计学意义。[结论]转移性乳腺癌患者原发灶PARP-1蛋白高表达者较低表达者含铂化疗疗效差。[Purpose] To investigate the expression of PARP-1 in primary tumor of metastatic breast cancer and its relation to therapeutic effects and prognosis of patients. [Methods] The clinicopathological data of 92 patients with metastatic breast cancer,who received palliative chemotherapy of platinum-containing regimens,were enrolled in the study. The expression of PARP-1 was detected by immunohistochemistry in the primary tumors. The relationship of the expression level of PARP-1 with the therapeutic effects and prognosis of patients were analyzed. [Results] The positive rate of PARP-1 expression was 73.20% in patients with metastatic breast cancer. Patients with higher expression of PARP-1 also had higher expression of CXCR4（P=0.003）,and were prone to have more internal organ metastasis than patients with lower expression of PARP-1（P=0.025）. Patients with high PARP-1 expression were less sensitive to platinum based chemotherapy than those with low PARP expression（P=0.000）. Disease free survival（DFS） and overall survival（OS） showed a decreasing trend in patients with high expression of PARP-1（P=0.076 and P=0.694）. [Conclusion]Higher expression of PARP-1 protein is associated with poorer therapeutic effect to platinum based chemotherapy in patients with metastatic breast cancer.
