泼尼松联合多西紫杉醇治疗前列腺癌临床疗效观察

摘　　要：目的观察泼尼松联合多西紫杉醇治疗晚期前列腺癌的临床效果。方法选择2010年2月—2015年12月收治的204例晚期前列腺癌,根据治疗方法分为观察组和对照组,每组102例。对照组采用多西紫杉醇,观察组在对照组基础上加泼尼松治疗。两组均以3周为1个疗程,3个疗程后随访12个月。比较两组临床疗效、前列腺特异抗原(PSA)及游离前列腺特异性抗原(f-PSA)水平、排尿及疼痛反应的评分、免疫功能及不良反应发生情况,评价生活质量。结果观察组总有效率高于对照组(P<0.05)。治疗后两组PSA及f-PSA水平低于治疗前,排尿症状及疼痛反应评分低于对照组,且观察组低于对照组(P<0.05)。观察组治疗后生理状况、情感状况、社会/家庭状况及功能状况优于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。治疗后两组CD3、CD4百分比高于治疗前,CD8百分比低于对照组,且观察组改善情况优于对照组(P<0.05)。结论泼尼松联合多西紫杉醇治疗晚期前列腺癌临床效果显著,且安全性较高。Objective To observe clinical effect of Prednisone combined with Docetaxel in treatment of patients with advanced prostate cancer. Methods A total of 204 patients with advanced prostate cancer admitted during February2010 and December 2015 were divided into observation group( n = 102) and control group( n = 102) according to treatment methods. Control group was treated with Docetaxel,while observed group was added with Prednisone on the basis of treatment for control group. In two groups,one course of treatment was 3 weeks,and all patients were followed up for 12 month after treatment for 3 courses. Clinical effect,level of prostate-specific antigen( PSA) and free prostate specific antigen( f-PSA),urination and pain response scores and incidence conditions of immune function and adverse reactions were compared,and life quality was evaluated in two groups. Results The total effective rate in observation group was significantly higher than that in control group( P < 0. 05). After treatment,PSA and f-PSA levels and urination and pain response scores were significantly lower than those before treatment in two groups,and the values in observation group were significantly lower than those in control group( P < 0. 05); physical condition,emotional condition,social/family and functional conditions in observation group were superior than those in control group( P < 0. 05). Incidence rate of adverse reactions in observed group was lower than that in control group( P < 0. 05). Compared with before treatment,after treatment,percentages of CD3 and CD4 were significantly increased,while CD8 percentages were significantly lower in two groups,and the improved conditions in observation group were superior than those in control group( P < 0. 05).Conclusion Prednisone combined with Docetaxel in treatment of patients with advanced prostate cancer had good effect and safety.
