氯吡格雷联合依达拉奉治疗脑梗死60例效果及相关血液流变学指标的变化

摘　　要：目的探讨氯吡格雷联合依达拉奉治疗脑梗死的临床效果。方法选取2015年1月—2016年1月武警后勤学院附属医院收治的脑梗死患者120例,按照治疗方法不同分为治疗组和对照组,每组60例。对照组应用氯吡格雷治疗,治疗组应用氯吡格雷联合依达拉奉治疗。比较两组的临床疗效、神经功能缺损情况、脑梗死病灶范围、血液流变学指标及同型半胱氨酸(Hcy)水平。结果治疗组临床总有效率和脑梗死病灶范围明显变小或变淡率高于对照组(P<0.05)。治疗后1周和2周,两组神经功能缺损评分均低于治疗前,且治疗组低于对照组(P<0.05)。治疗后,两组血浆黏度和血小板聚集率均低于治疗前,且治疗组低于对照组(P<0.05);治疗组血栓形成系数低于治疗前和对照组(P<0.05)。治疗后1周,治疗组血浆Hcy水平低于治疗前(P<0.05)。治疗后2周,两组血浆Hcy水平均低于治疗前,且治疗组低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论氯吡格雷联合依达拉奉治疗脑梗死临床效果较好,能够有效改善神经功能,缩小病灶范围,有助于恢复梗死区域血流灌注,改善患者预后。Objective To investigate clinical efficacy of Clopidogrel combined with Edaravone in treatment of patients with cerebral infarction. Methods A total of 120 patients with cerebral infarction admitted during January 2015 and January 2016 were randomly divided into treatment group( n = 60) and control group( n = 60). Control group was treated with Clopidogrel,while treatment group was treated with Clopidogrel combined with Edaravone. Clinical effect,defective conditions of nerval function,range of cerebral infarction lesions,hemorrheologic indexes and homocysteine( Hcy) levels were compared in two groups. Results The total effective rate and obviously decreased or thinning rate of cerebral infarction lesions range in treatment group were significantly higher than those in control group( P < 0. 05). In 1 and 2 weeks after treatment,defective scores of nerval function were significantly lower than those before treatment in two groups,and the scores in treatment group were significantly lower than those in control group( P < 0. 05). After treatment,blood viscosity and platelet aggregation rate were significantly lower than those before treatment in two groups,and the values in treatment group were significantly lower than those in control group( P < 0. 05). Thrombosis coefficient value in treatment group was significantly lower than those before treatment in same group and in control group( P < 0. 05).In 1 week after treatment,plasma Hcy level was significantly lower than that before treatment in treatment group( P <0. 05). In 2 weeks after treatment,plasma Hcy levels were significantly lower than those before treatment in two groups,and the level in treatment group was significantly lower than that in control group( P < 0. 05). There was no significant difference in incidence rate of adverse reaction between two groups( P > 0. 05). Conclusion Clopidogrel combined with Edaravone in treatment of patients with cerebral infarction may achieve good effectiveness,improve nerval function and decrease range of cerebral infarc
