摘 要:目的比较白内障超声乳化吸除联合植入多焦点人工晶状体(MIOL)眼与正常年轻人有晶状体眼视力、高阶像差(HOAs)及调制传递函数(MTF),分析多焦点人工晶状体眼患者的视觉质量。方法前瞻性病例对照研究。收集2016年5~10月就诊于我科的年龄相关性白内障术中植入多焦点IOL患者13例(21只眼),对照组为正常有晶状体眼的年轻人14例(26只眼)。术后3个月检查各组纳入眼的裸眼远视力(UCDVA)、裸眼近视力(UCNVA),最佳矫正远视力(BCDVA),视远矫正下的近视力(BCNVA)、屈光状态(球、柱镜度数)以及使用i TRACE全视觉功能分析仪测量角膜高阶像差(HOAs)及调制传递函数(MTF)。结果 MIOL组术后裸眼远视力与对照组裸眼远视力差异有统计学意义(P<0.05),但两组间裸眼近视力、最佳矫正远视力及远视矫正下近视力差异无统计学意义(P>0.05);MIOL组术后3个月球镜、柱镜度数与对照组差异无统计学意义(P>0.05);4 mm瞳孔下MIOL组全眼总高阶像差、球差、三叶草差差异无统计学意义(P>0.05),但慧差两组间差异有统计学意义(P<0.05);在5 10 15 20 25及30 cpd各空间频率下,MIOL组与正常年轻人对比差异均无统计学意义(P>0.05)。结论多焦点人工晶状体植入术后远、近全程视力、高阶像差及调制传递函数均接近正常年轻人眼水平,明显改善了白内障患者术后视觉质量。Objective To compare visual acuity,high order aberrations(HOAs) and modulation transfer function(MTF) in patients after cataract phacoemulsification and implantation of multifocal intraocular lens( MIOL) to normal young phakic eyes. Methods A prospective case-control study. Thirteen patients(21 eyes) received phacoemulsification and MIOL implantation( MIOL group). 26 normal young phakic eyes were enrolled as control. Three months after surgery,uncorrected distant and near vision,best-corrected distant vision,near vision with corrected distant vision,diopter,high order aberrations and modulation transfer function were recorded. The independent-samples t test was used to compare the data between patients and control subjects. Results There was no significant difference in the measurements between the patients and normal subjects except uncorrected distant vision was worse in the MIOL group( P〈0. 05). The coma aberration was higher in MIOL group( P〈0. 05). All other high order aberrations and diopters were similar to those of the normal subjects. No difference in MTF between the two groups. Conclusions Visual acuity,high order aberrations(HOAs) and modulation transfer function(MTF) in patients who received cataract phacoemulsification and multifocal intraocular lens implantation were very close to normal young phakic eyes. MIOL implantation improved the optical quality of cataract patients.
