小梁切除术联合抗代谢药物应用治疗新生血管性青光眼效果评价

摘　　要：目的探讨小梁切除术联合丝裂霉素、干扰素治疗新生血管性青光眼的应用价值。方法选择2013年3月至2015年3月我院收治的新生血管性青光眼172例(172只眼)为研究对象,随机将其分为对照组(n=86)及研究组(n=86),对照组实施小梁切除术进行治疗,研究组在对照组的基础上术中加用丝裂霉素与干扰素进行治疗,比较两组患者的治疗效果。结果术前,两组患者眼压比较无明显差异(P>0.05);研究组患者术后1周,术后3、6、12个月明显优于对照组(P<0.05)。术后12个月,两组患者功能性滤过泡形成、视力提高、新生血管全部回退例数均优于对照组(P<0.05)。研究组并发症发生率为10.4%,明显低于对照组并发症发生率31.3%(P<0.05)。结论新生血管性青光眼实施小梁切除术及丝裂霉素、干扰素联合治疗,可有效控制患者眼压,提高患者视力,促进患者新生血管消退,并发症少,值得推广及应用。Objective To investigate the value of glaucoma surgery（trabeculectomy） combined with mitomycin and interferon in the treatment of neovascular glaucoma. Methods Neovascular glaucoma patients（ n = 172 eyes） treated in our hospital between March 2013 and 2015 were involved in this study. Patients were randomly divided into control group（ n = 86） and experiment group（ n = 86）. Patients in the control group received glaucoma surgery（trabeculectomy） alone while patients in the experiment group were treated with mitomycin C and interferon in addition to the surgery. Results There was no significant difference in baseline IOP between the two groups. After treatment,IOP of the patients in the experiment group was significantly better than that of the patients in the control group（ P〈0. 05）. This difference was observed at 1 week and last until the end of 12-month follow-up. At 12 months,functional bleb formation,visual acuity,and the number of new vessels in all patients were significantly improved（ P〈0. 05）. The incidence of complications was 10. 4% in the experiment group,which was significantly lower than that in the control group（31. 3%; P〈0. 05. Conclusions Combined trabeculectomy and mitomycin C and interferon effectively controls IOP,improves visual acuity,promotes neovascularization subsided,causes fewer complications,and is worthy of promotion and application.
