摘  要:目的探讨肝动脉化疗栓塞(TACE)联合放射性粒子^(125)I组织间放疗对原发性肝癌(HCC)患者恶性生物学指标水平的影响。方法选取80例HCC患者并随机分为对照组和治疗组,各40例。对照组给予单纯TACE治疗,治疗组给予TACE联合放射性粒子^(125)I组织间放疗,治疗周期为6个月。治疗后检测并比较2组患者肿瘤生长相关分子、血清侵袭相关指标及肝癌标志性分子等恶性生物学指标水平。结果治疗后治疗组患者血管内皮生长因子(VEGF)、成纤维细胞生长因子(FGF)、金属蛋白酶(MMP)、细胞间粘附分子(ICAM-1)、内皮细胞特异性分子(ESM-1)、尿激酶型纤溶酶原激活物(uPA)、甲胎蛋白异质体(AFP-L3)、高尔基体蛋白73(GP73)、可溶性B7-H3(SB7-H3)、α-L-岩藻糖苷酶(AFU)、组织蛋白酶S(Cat S)水平均显著低于对照组(P<0.01)。结论 TACE联合放射性粒子^(125)I组织间放疗,能够有效抑制肝癌细胞的活性,促进其凋亡,降低肿瘤细胞恶性程度,延缓病情进展,安全可靠。Objective To explore the influence of transcatheter arterial chemoembolization (TACE) combined with radioactive particle ^125I interstitial radiotherapy on malignant biological indexes in patients with primary hepatocellular carcinomaon (HCC). Methods 80 cases of HCC patients were selected and randomly divided into the control group and the treatment group, 40 cases in each group. The control group was treated with TACE ,the treatment group was treated with TACE combined with radioactive particle ^125I interstitial radiotherapy, the treatment period was 6 months. After treatment, the tumor growth related molecules, serum invasion related indexes and malignant biomarkers of liver cancer markers were detected and compared between the 2 groups. Results Mter the treatment, the levels of vascular endothelial growth factor ( VEGF), fibroblast growth factor ( FGF), metalloproteinase ( MMP), intercellular adhesion molecule ( ICAM-1 ), endothelial cell specific molecule ( ESM-1 ), urokinase-type plasminogen activator( uPA), urokinase ( AFP-L3 ), Golgi protein 73 ( GP73 ), soluble BT-H3 ( SB7-H3 ), α-L-fucosidase ( AFU ), cathepsin S( Cat S) in the treatment group were significantly lower than those of the control group(P 〈 0.01 ). Conclusion TA- CE combined with radioactive particle ^125I interstitial radiotherapy can effectively inhibit the activity of liver cancer cells, promote its apoptosis, reduce the degree of malignant tumor ceils, delay the progression of the disease, and it is safe and reliable.
