摘 要:目的检测慢性牙周炎患者龈沟液(GCF)中白细胞介素-17(IL-17)和转化生长因子-β1(TGF-β1)的表达水平,探讨两者在慢性牙周炎发生发展中的作用。方法选择2015年6月~2017年5月宜宾市第一人民医院口腔科确诊的慢性牙周炎患者42例作为研究对象,根据口腔情况分为中重度慢性牙周炎组(A组,22例)和轻度慢性牙周炎组(B组,20例)。另选择同期牙周健康的体检者20例作为对照组。检测菌斑指数(PLI)、出血指数(BI)、探诊出血(BOP)、深度(PD)、附着丧失(AL)和龈沟液中IL-17和TGF-β1水平。结果 A、B组和对照组PD、BOP、BI、PLI和GCF的量比较,差异有统计学意义(P<0.05);A、B组龈沟液IL-17水平高于对照组(P<0.05);A组龈沟液IL-17水平高于B组(P<0.05)。B组龈沟液TGF-β1水平高于对照组和A组(P<0.05);A组龈沟液TGF-β1水平低于对照组(P<0.05)。患者GCF的量和龈沟液IL-17水平与PD、AL、BOP、BI、PLI呈正相关(r=0.712~0.869,P<0.05),患者龈沟液TGF-β1水平与PD、AL、BOP、BI、PLI呈负相关(r=-0.702^-0.839,P<0.05),患者龈沟液IL-17水平与TGF-β1水平呈负相关(r=-0.817,P<0.05)。结论 IL-17的表达与慢性牙周炎发生发展密切相关,可反映牙周组织破坏的程度;TGF-β1在慢性牙周炎局部组织中发挥免疫抑制作用。Objective To investigate the expression levels of interleukin-17(IL-17) and transforming growth factor beta 1(TGF-β1) in gingival crevicular fluid(GCF) in patients with chronic periodontitis, in order to explore the role of both in the development and progression of chronic periodontitis. Methods From June 2015 to May 2017, 42 patients with chronic periodontitis diagnosed in Department of Stomatology of the First People's Hospital of Yibin were selected as the subjects. They were divided into moderate and severe chronic periodontitis group(group A, 22 cases) and mild chronic periodontitis group(group B, 20 cases) according to the oral condition. Another 20 patients who had periodontal health at the same period were selected as control group. The plaque index(PLI), bleeding index(BI), diagnostic depth(PD),bleeding(BOP), probing depth(PD), loss of attachment(AL) and levels of IL-17 and TGF-β1 in gingival crevicular fluid were detected. Results There were significant differences in the levels of PD, BOP, BI, PLI and amount of GCF in the groups(P〈0.05). The levels of IL-17 in gingival crevicular fluid of group A and B were higher than that of control group(P〈0.05). The level of IL-17 in gingival crevicular fluid of group A was higher than that of group B(P〈0.05).The level of TGF-β1 in gingival crevicular fluid of group B were higher than that of control group and group A(P〈0.05). The level of TGF-β1 in gingival crevicular fluid of group A was lower than that of control group(P〈0.05). The amount of GCF and the level of IL-17 in gingival crevicular fluid were positively correlated with the levels of PD, AL,BOP, BI and PLI(r=0.712-0.869, P〈0.05).
