摘　　要：目的探讨进展期胃癌患者胃癌根治术中预防性腹腔热灌注化疗的疗效和安全性。方法进展期胃癌患者171例随机分为观察组(70例)和对照组(101例)。对照组行单纯胃癌根治术;观察组于胃癌根治术中给予顺铂+5-氟尿嘧啶腹腔热灌注化疗。记录两组术后相关指标,观察术后1年复发率和生存率。结果对照组和观察组术后并发症发生率、术后排气时间、手术前后血清C反应蛋白浓度变化、术后住院时间和术后肠梗阻发生率均无统计学差异(P>0.05)。观察组术后1年复发率稍低于对照组(P>0.05),术后1年生存率高于对照组(P<0.05)。结论胃癌根治术中预防性腹腔热灌注化疗治疗进展期胃癌安全性好,可有效提高生存率。Objective To explore the efficacy and safety of preventative intraperitoneal thermoperfusion chemotherapy during gastrectomy in the patients with advanced gastric cancer. Methods One hundred and seventy-one patients with advanced gastric cancer were randomly divided into two groups of A（70 cases） and B（101 cases）. The patients in group B received radical gastrectomy and those in group A received intraperitoneal thermoperfusion chemotherapy with cisplatin and 5-fluorouracil during surgery. The related indicators after operation were recorded and 1-year recurrence rate and survival rate were compared between two groups. Results There were no significant differences in the incidence of postoperative complications, postoperative exhausting time, the change of serum C-reactive protein concentration before and after operation, postoperative hospital stay and postoperative incidence of intestinal obstruction between two groups（P〉0. 05）. All patients were followed up for one year. The 1-year recurrence rate was slightly lower in group A than that in group B（P〉0. 05）. The 1-year survival rate was higher in group A than that in group B（P〈0. 05）. Conclusion Preventative intraperitoneal thermoperfusion chemotherapy during surgery is safe in the patients with advanced gastric cancer,which can increase survival rate.
