摘  要:目的探讨脑出血患者血清白细胞介素1β(interleukin-1 beta,IL-1β)、白细胞介素10(interleukin 10,IL-10)与周围脑组织水肿的相关性。方法选取2015-12/2016-12月作者医院收治的脑出血患者48例作为观察组,另选取50例同期体检健康人员作为对照组。采集所有研究对象外周血标本进行检测,主要观察IL-1β、IL-10指标水平,分析脑出血患者IL-1β、IL-10水平与患者出血量及周围组织水肿的相关性。结果观察组脑出血患者各时相IL-10水平均明显高于对照组(P均<0.001),但各时相观察组患者IL-1β指标与对照组比较无统计学差异(P>0.05);观察组脑出血患者各时相颅内血肿液IL-1β、IL-10指标均明显高于对照组脑脊液中IL-1β、IL-10水平(P<0.001或0.05),观察组患者发病后IL-1β、IL-10指标在24 h^3 d持续升高,3 d时达到顶峰,7 d、14 d时IL-1β、IL-10指标开始降低,但仍高于对照组;观察组脑出血患者发病后出血量、脑组织水肿量呈动态变化,并且在3 d时达到高峰值,发病6 h^3 d时患者周围脑组织水肿量与IL-10水平呈显著正相关关系(r=0.752,P<0.01)。结论脑出血患者血清IL-1β、IL-10可在一定程度上体现其病理过程,并且通过在不同时段评估IL-1β、IL-10水平对判断病情的严重程度具有意义。Objective To discuss the relationship in serum interleukin-1 beta(IL-1β ),interleukin 10(IL-10)and edema of surrounding brain tissue in patients with cerebral hemorrhage.Methods A total of 48 patients with cerebral hemorrhage and 50 health examined individuals in authors' hospital from December 2015 to December 2016 were selected,and divided into the observation group(n=48)and the control group(n=50).All the subjects were collected to examine the peripheral blood samples.And the levels of IL-1β and IL-10,the correlation between IL-1β ,IL-10 levels and the surrounding tissue edema in patients with cerebral hemorrhage were discussed.Results Compared with the control group,there were statistically different in the levels of IL-10 at each time points of two groups(P0.001),but there had no significant difference in the levels of IL-1β at each time points(P0.05).The levels of IL-1β and IL-10 in the intracranial hematoma fluid of observation group were significantly higher than those in the cerebrospinal fluid of control group(P0.001 or 0.05),the levels of IL-1β and IL-10 in the observation group were significantly increased from 24 h to reach apeak at 3 d,IL-1β and IL-10 began to decrease at 7 d and 14 d,but still higher than those in the control group.The amount of blood loss and brain tissues edema in patients with cerebral hemorrhage showed dynamic changes,and the peak value reached at 3 d,there was a significant positive correlation between the amount of brain tissue edema and the levels of IL-10 within6 h-3 d(r=0.752,P0.01).Conclusion To a certain extent,it can reflect the pathological process by serum IL-1β ,IL-10 in patients with cerebral hemorrhage,and determine the severities of the condition by assessing the levels of IL-1β and IL-10 at different times.
