疏血通联合血液净化治疗对高脂血症性重症急性胰腺炎患者高凝血状态的影响

摘　　要：目的研究疏血通联合血液净化治疗对高脂血症性重症急性胰腺炎(HL-SAP)患者高凝血状态的影响。方法选取我院2014年2月至2016年2月HL-SAP患者86例,随机抽签分为观察组与对照组两组,每组43例,给予对照组血液净化治疗,观察组在对照组治疗基础上联合疏血通治疗,比较两组患者治疗前后凝血酶原时间(PT)、部分凝血酶原时间(a PTT)、凝血酶时间(TT)、纤维蛋白原(Fbg)等凝血指标水平变化,全血高切黏度、全血低切黏度、血小板聚集率、血浆比黏度、红细胞沉降率等血液流变学指标比较两组患者机械通气时间、ICU住院时间、慢性健康状况评估系统APACHE II评分及生存率。结果治疗后,观察组PT、a PTT、TT上调,Fbg显著减低(P<0.05);观察组全血高切黏度、全血低切黏度、血小板聚集率、血浆比黏度、红细胞沉降率较对照组均显著较低(P<0.05);观察组患者机械通气时间、ICU住院时间、APACHE II评分较对照组显著较低(P<0.05),患者死亡率为6.98%较对照组25.58%显著较低(P<0.05)。结论疏血通联合血液净化治疗HL-SAP患者高凝状态得到改善,患者死亡率降低。Objective To study the effect of Shuxuetong combined with blood purification treatment on hypercoagulable state and laboratory related indexes in patients with hyperlipidemic severe acute pancreatitis （HL-SAP）. Methods 86 patients with HL-SAP admitted in our hospital between February 2014 and February 2016 were randomized into the observation group and the control group,43 cases in each group. The control group was given blood purification treatment while the observation group, on the basis of the treatment in control group, was treated with Shuxuetong. The changes of prothrombin time （ PT）, partial prothrombin time （aPTT）, thrombin time（ TY）, fibrinogen（Fbg） and other blood coagulation indexes before and after treatment, high shear whole blood viscosity, low shear whole blood viscosity, platelet aggregation rate, plasma viscosity, erythrocyte sedimentation rate and duration of mechanical ventilation, length of ICU stay, chronic health condition assessment system APACHE II score and survival rate were compared between the two groups. Results After treatment, in the observation group, PT, aPTT, TT were up-regulated, and Fbg was significantly decreased （P 〈 0. 05）. low shear whole blood viscosity, platelet aggregation rate, plasma viscosity, erythrocyte sedimentation rate（P 〈0. 05） ;the duration of mechanical ventilation, length of ICU stay and APACHE II score in the observation group were significantly shorter and lower than those in control group（ P 〈 0. 05 ） ;The mortality rate in observation group（6. 98% ）was significantly lower than that in control group（25.58% ） （p 〈 0. 05 ）. Conclusion The hypercoagulable state of patients with HL-SAP treated by Shuxuetong combined with blood purification was improved, and the mortality of patients was decreased.
