摘 要:目的:探讨广东贵屿电子垃圾拆解区帕金森病患者重金属暴露水平与病情严重程度的相关性。方法:选取2014年1月至2016年12月汕头大学第一附属医院门诊及住院的帕金森病患者20例为暴露组,患者均来自广东贵屿电子垃圾拆解区;同时选取本地区非电子垃圾拆解区的正常健康者20例为对照组。采用电感耦合等离子体质谱技术(ICP-MS)检测两组对象血锰、血铅和血铜水平。采用Spearman法分析该地区血锰、血铅和血铜水平与帕金森病病情严重程度的相关性。结果:暴露组患者血锰水平显著高于对照组,差异具有统计学意义(P<0.01),而两组血铅和血铜水平比较,差异均无统计学意义(P>0.05)。拆解区帕金森病晚期患者血锰水平显著高于早中期患者,差异具有统计学意义(P<0.05),而血铅和血铜水平比较,差异均无统计学意义(P>0.05)。Spearman分析显示拆解区患者血锰水平与患者Hoehn-Yahr(H-Y)分期呈明显正相关(r=0.626,P<0.01)。结论:广东贵屿电子垃圾拆解区帕金森病患者血锰水平明显升高,且晚期患者明显高于早中期患者。重金属暴露水平特别是血锰水平与患者病情严重程度呈正相关。Objective To investigate the correlation between heavy metal exposure and Parkinson disease in electronic waste dismantling area. Methods From January 2014 to December 2016, a total of 20 patients with Parkinson disease who were come from electronic waste dismantling area in were selected as the exposed group. At the same time, 20 healthy volunteers in non-electronic waste dismantling area were selected as the control group. The levels of blood manganese, blood lead and blood copper in the two groups were detected by inductively coupled plasma mass spectrometry(ICP-MS). The correlation of blood manganese, blood lead and blood copper levels with the severity of Parkinson disease were analyzed. Results The level of blood manganese in the exposed group was signifi cantly higher than that in the control group(P〈0.05), but there was no difference in blood lead and blood copper between the two groups(P〉0.05). The level of blood manganese in advanced stage patients was signifi cantly higher than that in early and middle stages patients(P〈0.05), but there was no difference in blood lead and blood copper(P〉0.05). Spearman analysis showed that there was a positive correlation between the blood manganese level and the H-Y staging(r = 0.626, P〈0.01). Conclusion The level of blood manganese in patients with Parkinson disease is signifi cantly increased in electronic waste dismantling area. And the advanced stage patients are signifi cantly higher than those in early and middle stages patients. Heavy metal exposure levels, especially blood manganese levels, are positively correlated with the severity of the disease.
