多途径介入治疗肝癌的疗效分析

摘　　要：目的　探讨多途径介入治疗肝癌 ,并对其疗效加以分析。方法　 34个肝癌病灶 (2 6例 )的多途径介入性治疗在超声引导下进行。 <3cm 2 0个病灶 ,行局部微波治疗 +门静脉化疗 ;>3cm 14个病灶 ,12个病灶行局部无水酒精注射治疗 +门静脉化疗 ,2个病灶行局部微波治疗 +无水酒精注射治疗 +门静脉化疗 ;7例合并门静脉癌栓者 ,加做门静脉癌栓内无水酒精注射治疗。结果　随访 12～ 2 5个月 ,一年生存率 10 0 % ,两年生存率 88.4 2 %。微波治疗组均经活检证实完全性坏死 ;无水酒精注射治疗组 U S及 CT报告病灶稳定。门静脉化疗预防组无癌栓出现 ;治疗组癌栓 4例缩小 ,3例消失。　结论　多途径介入治疗对预防肝癌转移 ,延迟复发 ,改善预后有肯定意义 。Objective To investigate the effect of multi methods interventional treatment for hepatic cancer.Methods All interventional treatments were operated with US guidance in 34 lessions of 26 patients with hepatic cancer.20 lesions(lesion diameter<3 cm) were given the treatments of local microwave coagulation combined with chemotherapy via portal vein.Among 14 lessions(lesion diameter>3 cm),12 lessions were given the treatments of injection of absolute alcohol combined with chemotherapy via portal vein,2 lessions were given local microwave coagultion combined with injection of absolute alcohol and chemotherapy via portal vein.7 cases of hepatic cancer companied with portal vein tumor thrombus were given injection of absolute alcohol for tumor thrombus in portal vein.Results All patients had been followed up for 6 25 months.The first year survival rate was 100% and the second year was 88.42%.The complete necrosis was proved by biopsy in the lessions with the treatment of local microwave coagulation.US and CT showed that the lesion cured by injectied absolute alcohol hadn't developed.After chemotherapy via portal vein,tumor thrombus didn't appeared in the preventive group,it disappeared in 3 cases and shrank in 4 cases in the treatment group.Conclusion The multi interventional treatments are of certain value in preventing metastasis,improving prognosis and prolonging the period of survival for hepatic cancer.
