摘  要:目的:将三维打印系统与放疗设计系统相结合,指导放射性粒子内照射治疗上颌、颅底腺样囊性癌,探索更高精度植入^(125)I放射粒子的方法,从而更加安全有效地控制已累及颅底及眶尖周的口腔颌面部恶性肿瘤。方法:肿瘤侵及颅底、上颌的腺样囊性癌患者8例,术前进行增强CT检查,进行放疗设计并制作三维打印模板。术中在模板引导下进行手术。结果:术中避免了颅脑及眶内组织损伤。实现了放射粒子组织内均匀分布,定位准确,无一例出现颅脑损伤及视力影响,术中出血少,治疗有效率(response rate,RR)达100%,疗效确切。结论:三维打印模板辅助下进行颅底眶尖周放射粒子植入是精确、安全的,有条件的情况下可考虑采用。Objective: The aim of this study was to find a more precise and safe way to implant radioactive particles into the maxilary and skull base regions using the three dimensional printing navigation system. Methods: Eight patients with invasive adenoid cystic carcinoma of the cranial base and maxilary and without any history of surgery were selected. A preoperative computed tomography scan was performed and three dimensional printing template was designed to aid the surgery. Results: No intracranial injury or visual impairment occurred, and the response rate was 100%. Conclusion: This study indicates that the implantation of radioactive particles using the three dimensional printing template was an effective and safe method for treating tumors in the skull base and maxilary, and should be considered for clinical use.
