自酸蚀黏结剂预处理对窝沟封闭效果的影响

摘　　要：目的观察自酸蚀粘结剂预处理对窝沟封闭效果的影响。方法选择2014年5月-2016年5月医院口腔科就诊的儿童患者102例,根据治疗方法分为观察组(51例共75颗牙)和对照组(51例共75颗牙)。对照组采用传统磷酸酸蚀法对牙面处理后用窝沟封闭剂对窝沟进行PFS;观察组用磷酸酸蚀后先用自酸蚀粘结剂预处理,再用窝沟封闭剂行PFS。对比2组治疗后的脱落率、操作时间及龋病发生率等。结果 2组患者6个月后窝沟封闭剂部分脱落及完全脱落对比差异无统计学意义(P>0.05)。观察组完全脱落率及总脱落率明显低于对照组(P<0.05)。观察组PFS一颗牙及全部牙的时间均明显长于对照组(P<0.05)。窝沟封闭6个月后观察组龋病发生率为0,对照组为1.33%(1/75)(P>0.05)。而窝沟封闭12个月后观察组龋病发生率为2.67%(2/75)明显低于对照组12.00%(9/75)(P<0.05)。结论窝沟封闭前涂布自酸蚀粘结剂预处理虽然操作时间较长,但不仅能有效降低窝沟封闭剂脱落率,而且可达到减少龋病发生率、预防龋病发生的目的,值得临床推广。Objective To study the effects of preconditioning with self-etching adhesive on pit and fissure sealant.Methods Selected 102 cases of children,a total of 150 molar teeth from May 2014 to May 2016 in the hospital. According to the different treatments they were randomly divided into observation group and control group,each of 51 cases,75 teeth. Control group was treated with traditional method with 35% phosphoric acid,while observation group was preconditioned with self-etching adhesive. Compared the losing ratio,operation time and the incidence of caries. Results There was no significant difference in partial shedding and complete shedding between two groups after 6 months（P〈0. 05）. The rate of complete exfoliation and the total exfoliation rate in observation group were significantly lower than those in control group,with significant differences（P〈0. 05）. The time of one tooth and all the teeth of PFS in observation group were significantly longer than that of control group（P〈0. 05）. The incidence of dental caries was 0 in observation group after 6 months of pit and fissure occlusion,while control group was 1. 33%（1/75）（P〈0. 05）. However,the incidence of dental caries was 2. 67%（2/75） in observation group after 12 months of fissure septal occlusion,which was significantly lower than that in control group（12. 00%,9/75）（P〈0. 05）. Conclusion Although it required more time to precondition with self-etching adhesive,it can effectively reduce the losing ratio of the PFS and the incidence of caries,which is worthy of clinical promotion.
