胃镜电凝止血联合镜下蒙脱石散创面给药对胃溃疡出血患者溃疡恢复和疼痛程度的影响

摘　　要：[目的]观察胃镜电凝止血联合镜下蒙脱石散创面给药对胃溃疡出血患者溃疡恢复和疼痛的影响。[方法]选取因胃溃疡出血药物治疗效果不佳需胃镜下电凝止血患者104例,随机分为观察组、对照组,每组52例。观察组予以胃镜电凝止血联合镜下蒙脱石散创面给药治疗,对照组仅予以胃镜下电凝止血,比较2组治疗后临床效果、溃疡愈合、疼痛程度、止血时间、输血量、住院时间、再出血以及不良反应情况。[结果]观察组治疗后痊愈率和总有效率高于对照组,差异具有统计学意义(P<0.05);观察组治疗后愈合率高于对照组,溃疡愈合时间少于对照组,差异具有统计学意义(P<0.05);观察组治疗后0级和1级疼痛率高于对照组,2级和3级疼痛率低于对照组,疼痛评分低于对照组,差异具有统计学意义(P<0.05);观察组治疗后止血时间、输血量和住院时间均少于对照组,差异具有统计学意义(P<0.05);观察组治疗后30d内再出血率低于对照组,总再出血率低于对照组,差异具有统计学意义(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。[结论]胃镜电凝止血联合镜下蒙脱石散创面给药治疗胃溃疡出血临床效果较好,术后恢复快,疼痛感轻,溃疡愈合快,再出血率低,推荐临床广泛推广应用。[Objective]To investigate the effect of montmorillonite on the recovery of ulcer bleeding and pain relief in patients with gastric ulcer bleeding. [Methods]From March 2014 to October 2016,104 cases accepting endoscopic coagulation treatment for gastric ulcer bleeding patients after the failed medicine treatment were selected. The patients were randomly divided into two groups according to the admission sequence,each group had 52 cases. They were treated with gastroscopic coagulation combined with monoclonal stone powder or only gastroscopic eleetrocoagulation hemostasis treatment. The ulcer healing, pain relief, hemostasis, blood transfusion, hospital stay, rebleeding and adverse reactions between the two groups after treatment were compared. [Results]The cure rate and the total effective rate in the observation group were higher than those in the control group, the difference was statistically significant（P〈0.05）. The healing rate of the observation group was higher than that of the control group, the healing time of ulcer was less than that of the control group, the difference was statistically significant（P〈0.05）. The pain rate of grade 0 and grade 1 in the observation group was higher than that in the control group, the pain rate of grade 2 and 3 was lower than that of the control group, the pain score was lower than that of the control group,the difference was statistically significant（P〈0.05）. The time of hemostasis, blood transfusion and hospital stay were lower in the observation group than in the control group, the difference was statistically significant（P〈0.05）. The rebleeding rate in the observation group was lower than that in the control group within 30 days after treatment,the total rebleeding rate was lower than that of the control group, the difference was statistically significant（P〈0.05）.
