血清白蛋白、前白蛋白水平对非小细胞肺癌患者预后的影响

摘　　要：目的探讨非小细胞肺癌患者血清白蛋白(ALB)、前白蛋白(PAB)水平与非小细胞肺癌预后的关系。方法选取2011年8月至2013年3月就诊于我院确诊的非小细胞肺癌患者210例,选择60例肺部良性疾病患者作为对照,采用生化分析仪检测NSCLC患者和良性组患者血清ALB、PAB表达量,应用χ2检验分析血清ALB、PAB表达量与NSCLC患者临床病理参数的关系;采用Kaplan-Meier分析血清ALB、PAB表达量与NSCLC总生存期(OS)的关系。结果血清ALB、PAB在NSCLC中的阳性表达量均显著低于良性组患者(P<0.05)。血清ALB、PAB与NSCLC临床分期、淋巴结转移相关(P<0.05)。生存分析显示血清ALB、PAB阳性患者生存期显著高于阴性患者(P<0.05)。多因素COX回归显示NSCLC临床分期、淋巴结转移、血清ALB、PAB是预后的独立影响因素。结论血清ALB、PAB与NSCLC临床分期以及淋巴结转移有关,血清ALB、PAB阴性表达提示NSCLC患者OS较短,有望作为NSCLC预后预测的分子标志物。Objective To investigate the prognostic value of serum ALB and PAB in patients with non-small cell lung cancer. Methods The clinical data of 210 patients from August 2011 to March 2013 with non- small cell lung cancer were retrospectively analyzed, and 60 patients with benign lung diseases were selected as controls group. Expression levels of serum ALB and PAB were detected by biochemical analyzer. The relationship between the expression of serum ALB and PAB and the clinicopathological parameters of NSCLC patients were analyzed through X2 test, The relationship between the expression of serum ALB and PAB and the total survival time （ OS ） of NSCLC was analyzed by Kaplan- Meier. Results The expression levels of serum ALB and PAB in NSCLC patients were significantly higher than those in the control group （ P 〈 0.05 ）. The expression levels of serum ALB and PAB in NSCLC were correlated with TNM stage and lymph node metastasis（ all P 〈0.05）. Kaplan-Meier analysis showed that low expression levels of serum ALB and PAB were closely associated with a short survival time （ P 〈 0.05 ）. Cox proportional hazard regression showed that the expressions of serum ALB and PAB were independent risk factor affecting the proportional of NSCLC （ P 〈 0.05 ）. Conclusion The expressions of serum ALB and PAB were high in NSCLC patients, and may be expected to be a molecular marker for predicting the prognosis of NSCLC.
