针刺治疗脑卒中后抑郁的临床观察

摘　　要：目的:观察针刺百会、内关、太冲治疗脑卒中后抑郁的临床疗效。方法:将符合标准的60例脑卒中后抑郁患者随机分为观察组和对照组各30例,两组患者均进行基础治疗,观察组在此治疗的基础上加用针刺治疗,主穴为百会、内关、太冲,对照组则加用西药盐酸舍曲林片进行治疗,治疗均持续4周。两组患者在治疗前后均采用汉密尔顿抑郁量表(HAMD)评定其抑郁程度,采用HAMD减分率评价其临床疗效。结果:观察组和对照组患者治疗后的HAMD总分评分较治疗前均有明显下降,差异有统计学意义(P<0.05)。治疗后观察组和对照组的临床疗效相当,差异无统计学意义(P>0.05)。结论:针刺百会、内关、太冲对治疗脑卒中后抑郁有明显的疗效,能使患者的抑郁症状得到有效改善,其临床疗效与舍曲林相当。Objective： To observe the clinical effect of acupuncture in Baihui, Neiguan and Taichong on post-stroke depression. Methods： 60 patients with post-stroke depression were randomly divided into the observation group and the control group, 30 cases in each group. Two groups of patients were treated with basic treatment, the observation group on the basis of this treatment plus acupuncture treatment, the main point is Baihui, Neiguan,Taochong, the control group was treated with western medicine sertraline hydrochloride tablets. The patients in both groups were evaluated by Hamilton Depression scale（Hamd） before and after treatment, and the clinical efficacy was evaluated by HAMD score reduction rate. Results： the total score of HAMD in the observation group and the control group decreased significantly compared with that before treatment, and the difference was statistically significant（P〈0.05）. After treatment, the clinical efficacy of the observation group and the control group was equivalent, the difference was not statistically significant（P〈0.05）. Conclusion： The acupuncture Baihui, Neiguan and Taichong have obvious curative effect on the treatment of post-stroke depression, can effectively improve the depressive symptoms of the patient, and the clinical curative effect is equivalent to that of the sertraline.
