恶性肿瘤患者围术期输血策略应用的随机对照临床研究

摘　　要：目的通过研究评估围术期输血指征评分的输血方案在恶性肿瘤患者应用的可行性和安全性。方法通过随机、对照、临床研究对比接受不同输血指南治疗的恶性肿瘤患者预后情况结果,记录患者术前、术中、术后指标以评估患者预后。结果本研究最终全国各中心共纳入恶性肿瘤受试者442例。术后短期死亡率及并发症发生率、肿瘤复发发生情况比较受试者的死亡率没有统计学差异。POTTS指导组在整个围术期输血量也低于其余两个受试组,有统计学意义。结论 POTTS输血方案对于恶性肿瘤的应用具有安全性及有效性,不增加肿瘤复发转移和死亡率。Objective To perspectively analyze the safety and feasibility of Perioperative Transfuseon Trigger Score system according to this clinical study. Methods This research was a prospective,multicenter,randomized study. The clinical date of patients were recorded to fully analyze the outcome of malignant tumor patients undergoing malignant tumor surgery.Results 442 cases of malignant tumor patients were tested. Postoperative short-term mortality,death rate,metastasis and recurrence rate were comparable of the 3 groups. The blood transfusion rate and amount were much lower in the POTTS group than the other 2 groups. Conclusion POTTS blood transfusion can reduce perioperative red blood cell transfusion and infusion costs,while not improve the clinical mortality rate or metastasis.
