182例EB病毒感染患儿疾病谱分析

摘　　要：目的探讨小儿EB病毒(EBV)感染疾病谱分布。方法选取我院2012年1月至2016年12月收治的182例儿童EBV感染患儿为研究对象,根据患儿的年龄分为婴儿组(<1岁)、幼儿组(1~3岁)、学龄前期组(3~6岁)和学龄组(>6岁)共四组,统计各组患儿疾病谱分布情况。结果 182例EBV感染患儿中,男性患儿98例,女性患儿84例;男女比例1.17∶1;感染例数:学龄前组>幼儿组>学龄组>婴儿组。主要疾病谱所占比例从高到低依次为传染性单核细胞增多症(48.9%)、呼吸道感染(33.5%)、过敏性紫癜(6.0%)、消化系统疾病(3.8%),其他少见有中枢异常(1.6%)、血液系统疾病(1.6%)、泌尿系统疾病(1.6%)、颈部淋巴结炎(1.6%)、面神经炎(1.1%)等。结论儿童EBV感染率高,临床表现多样,累及多系统多器官,病情轻重不一,给临床诊断和治疗造成困难,儿科医师应重视EBV感染相关的疾病谱,早期识别、早期诊断、避免误诊、漏诊,使患儿及时得到有效治疗。Objective To analyze the Epstein-Barr virus（EBV） infection disease spectrum of children. Methods A total of 182 cases of children with EBV infection admitted in our hospital from January 2012 to December 2016 were selected as the research objects. All the children were classified into four groups： infant group（1 years old）, young children group（1-3 years old）, preschool group（3-6 years old） and school age group（6 years old）. Disease spectrum in each age groups were summarized. Results In 182 children with EBV infection, 98 cases were male and 84 cases were female, the proportion of men and women was 1.17∶1. The number of infection cases： preschool group young children group school age group infant group. The proportion of major diseases ranged from high to low： infectious mononucleosis（48.9%）,respiratory tract infection（33.5%）, henoch-schonlein purpura（6.0%）, digestive system diseases（3.8%）, and other rare as central abnormalities（1.6%）, hematological diseases（1.6%）, urinary system diseases（1.6%）, neck lymphadenitis（1.6%） and facial neuritis（1.1%）. Conclusion The infection rate of EBV in children is high, the clinical manifestations are diverse, the multiple systems and organs are involved, and the severity of the disease is different, which makes it difficult for clinical diagnosis and treatment. The pediatrics doctors should pay close attention to EBV infection related diseases, and give early identification and diagnosis, so as to avoid misdiagnosis and missed diagnosis, so that the children can receive effective treatment in time.
