摘  要:目的观察经23G微创切口睫状体光凝术治疗硅油眼继发性青光眼的临床效果。方法前瞻性研究14例(14眼)硅油眼继发性青光眼应用最大量的抗青光眼药物仍无法控制眼压,采用经23G微创切口进行睫状体激光光凝术,观察术后1d、7d、1个月和3个月的眼压、并发症及辅助抗青光眼药物使用情况。结果11例术后各观察时问点眼压均在正常范围,3例术后7d开始眼压升高,应用降眼压药物后控制正常。术前术后眼压对比差异有统计学意义(P〈0.05)。并发症主要为前房积血和较重的炎症反应,均经药物治疗而得到控制;术前、术后辅助用抗青光眼药物使用数量比较,差异有统计学意义(P〈0.05)。结论硅油取出术后眼压仍无法控制的患者,经23G微创切口行睫状体直接光凝,可有效控制眼压。Objective To observe the clinical efficacy of direct cyclophotocoagulation through 23 G minimal incision for secondary glaucoma of silicone oil eyes. Methods A prospective study. Fourteen eyes of 14 cases with secondary glaucoma of silicone oil eyes received the application of the maximum anti- glaucoma drugs, but the intraocular pressure (IOP) weren' t controlled. They underwent direct cyclophotocoagulation through 23 G minimal incision. The IOP, complications and usage of anti-glaucoma drugs were observed at 1 day, 7 days, 1 and 3 months after treatment. Results The IOP after treatment were normal in 11 patients at every follow-up point. The IOP was increased in 3 cases at 7 days after operation and were controlled after anti-glaucoma drugs treatment. The difference of IOP was statistically significant between the pre- and post-operation ( P 〈 0.05 ). The complications were hyphema and severe inflammatory reaction. The difference in the number of anti-glaucoma drugs was significant between pre-and post-operation (P 〈 0.05 ). Conclusion It is an effective method to control IOP by direct cyclophotocoagulation through 23 G minimal incision for intraocular hypertension after silicone oil removal.
