联合TTF-1、CK7、P63、CK5／6、CD56在肺癌组织学分型中的意义

摘　　要：目的探讨联合TTF-1、CK7、P63、CK5／6、CD56免疫组化在肺癌诊断及组织学分型中的意义。方法选取587例肺癌标本，进行免疫组化染色，所选抗体包括：TTF-1、CK7、P63、CK5／6、CD56。结果TTF-1、CK7在诊断腺癌的灵敏度和特异度分别为87．5％、97．6％和43-3％、89．2％，两者联合诊断肺腺癌的灵敏度和特异度分别为85．6％和99．2％；P63、CK5／6在诊断鳞癌的灵敏度和特异度分别为97．0％、97．6％和91．2％、95．0％，两者联合诊断肺鳞癌的灵敏度和特异度分别为94．6％和100．0％；TTF-1、CD56在诊断肺小细胞神经内分泌癌的灵敏度和特异度分别为90．1％、98．1％和43．3％、98．9％，两者联合诊断肺小细胞神经内分泌癌的灵敏度和特异度分别为88．3％和100．0％。结论联合TTF-1、CK7、P63、CK5／6、CD56免疫组化在确定肺癌组织学分型中意义重大。Objective To investigate the diagnostic value of TTF-1, CK7, P63, CK5/6, and CD56 in the histological typing of lung cancer. Methods The expressions of TTF-1, CK7, P63, CK5/6, and CD56 were detected by immunohistochemical method in 587 cases of lung cancer. Results The sensitivities and specificities of TTF-1 and CK7 in adenocarcinoma were 87.5% and 97.6% and 43.3% and 89.2%, respectively. The sensitivity and specificity of the combination of TTF-1 and CK7 in adenocarcinoma were 85.6% and 99.2%. The sensitivities and specificities of P63 and CK5/6 in squamous cell carcinoma were 97.0% and 97.6% and 91.2% and 95.0%. The sensitivity and specificity of the combination of P63 and CK5/6 in Squamous cell carcinoma were 94.6% and 100.0%. The sensitivities and specificities of TTF-1 and CD56 in small cell lung cancer were 90.1% and 98.1% and 43.3% and 98.9%, respectively. The sensitivity and specificity of the combination of TTF-1 and CD56 in small cell lung cancer were 88.3% and 100.0%. Conclusions TTF-1, CKT, P63, CK5/6, and CD56 have great significance in the histological typing lung cancer.
