脑卒中高危人群体质指数与颈动脉斑块的关系研究

摘　　要：目的探讨体质指数(BMI)与脑卒中高危人群颈动脉斑块之间的相关性。方法利用浙江省人民医院登记的546例脑卒中高危患者的资料,分析其BMI与颈动脉斑块发生的关系,并应用logistic回归分析不同BMI合并影响脑卒中的危险因素对形成颈动脉斑块的危险。结果 logistic回归分析显示,BMI是脑卒中患者颈动脉粥样斑块病变的独立危险因素(OR=1.653,95%CI:1.063~2.572,P=0.026)。超重组以及肥胖组合并脑卒中的相关危险因素包括年龄和高血压,糖尿病组颈动脉斑块的形成率均高于对照组(P<0.05)。其中,合并高血压病史组的颈动脉粥样斑块的形成率均显著高于无高血压组,分别是对照组的2.270、3.167和3.647倍(P<0.01)。结论随着BMI升高,特别是具有脑卒中相关危险因素的人群,发生颈动脉粥样斑块的风险也明显升高。Objective To investigate the relationship between body mass index （BMI） and carotid plaque in population at high risk for stroke. Methods The clinical data of 546 cases at high risk for stroke in our hospital were analyzed. Color Doppler was used to examine the number of carotid plaque. The correlation between BMI level combined with other risk factors and carotid plaque. Results High BMI was the independent risk factor for carotid plaque formation （ OR = 1.653, 95% CI： 1.063-2.572, P =0.026） . The formation rates of carotid plaque in overweight and obesity groups with risk factors for stroke, including age and hypertension, and diabetes group were all higher than that in control group （all P〈0.05） . Especially, the formation rate of carotid plaque in group complicated with hypertension were 2.270, 3.167 and 3.647 times higher than those in control group respectively （all P〈0.01）. Conclusion Elevated BMI is associated with carotid plaque formation, especially in those complicated with hypertension, dysglycemia, at risk age and with other risk factors.
