摘  要:目的探讨肿瘤蛋白P53、凋亡抑制蛋白livin、谷胱甘肽转移酶-π(GST-π)和Ⅱ型DNA拓扑异构酶(DNA TopoⅡ)在上皮性卵巢癌中的表达情况并分析其临床意义。方法选取经病理学检查确诊的82例上皮性卵巢癌患者,取其上皮性癌变组织作为研究对象,根据复发情况分为化疗敏感组和化疗耐药组。采用免疫组化SP法检测化疗耐药蛋白P53、livin、GST-π和TopoⅡ的表达情况,分析其与临床病理特征的关系。结果化疗敏感组P53、livin和GST-π的阳性表达率均明显低于化疗耐药组(P﹤0.01),而TopoⅡ的阳性表达率高于化疗耐药组(P﹤0.05);化疗敏感组5年累积生存率(58.3%)高于化疗耐药组(12.5%,P﹤0.05)。临床分期和化疗敏感程度是生存时间的影响因素(P﹤0.05)。结论 P53、livin、GST-π和TopoⅡ可以作为上皮性卵巢癌患者化疗效果和预后效果的预测指标,根据其表达情况调整治疗方案,改善患者预后。Objective To investigate the expression of tumor protein 53(P53),livin,glutathione transferase-π(GST-π)and DNA topoisomerase Ⅱ(Topo Ⅱ) in epithelial ovarian cancer and discuss the clinical significance.Method A total of82 patients with pathologically confirmed epithelial ovarian cancer were included in the study,from which the cancer tissues were collected,according to the status of recurrence,the patients were divided into chemotherapy-sensitive group and chemotherapy-resistant group.Immunohistochemical SP method was used to detect the expression of P53,livin,GST-πand Topo Ⅱ,and the relationship with clinicopathological features was analyzed.Result The positive expression rate of P53,livin and GST-π in chemotherapy-sensitive group was significantly lower than that in drug-resistant group(P〈0.01),while the positive expression rate of Topo Ⅱ in chemotherapy-sensitive group was significantly higher than that in chemotherapy-resistant group(P〈0.05); the 5-year survival rate of chemotherapy-sensitive group(58.3%) was significantly higher than that of chemotherapy-resistant group(12.5%,P〈0.05).Clinical stage and chemotherapy sensitivity were factors influencing the survival time(P〈0.05).Conclusion P53,livin,GST-π and Topo Ⅱ can be used as predictors of the clinical effect of chemotherapy and the prognosis in patients with epithelial ovarian cancer,for which the treatment regimens can be modified according to their expression to improve the prognosis of patients.
