胸段食管癌、贲门癌淋巴结转移特点及影响因素分析

摘　　要：目的探究影响胸段食管癌、贲门癌淋巴结转移的临床因素。方法回顾性分析65例经手术治疗的胸段食管癌、贲门癌患者临床资料,分析胸段食管癌、贲门癌各区域淋巴结转移情况,采用单因素分析及Logistic回归分析病理因素对淋巴结转移的影响。结果胸段食管癌以上纵隔、肺门,隆突下较为多见;贲门癌转移主要以腹腔、贲门周围多见。对患者年龄、肿瘤细胞病理类型,肿瘤浸润深度,长度,分化程度等进行单因素分析,结果提示肿瘤浸润全层,长度超过5 cm以及细胞分化程度低、位于胸下段的肿瘤和淋巴清扫个数>3与淋巴结转移相关(P<0.05)。将上述有意义的单因素纳入多因素Logistic回归分析,结果提示,肿瘤分化程度,长度,浸润深度为淋巴结转移的独立影响因素(P<0.05)。结论食管癌、贲门癌的淋巴结转移是具有一定的特点,术前全面合理的检查,根据肿瘤分化程度,病变长度,浸润深度等实际情况制定淋巴结清扫范围。Objective To analyze the lymph node metastasis of thoracic esophageal and cardiac cancer, and to explore the clinical factors influencing the metastasis. Methods The clinical data of 65 patients with thoracic esophageal and cardiac cancer were analyzed retrospectively. The lymph node metastasis of thoracic esophageal and cardiac cancer was analyzed. Univari- ate analysis and logistic regression analysis were used to analyze the changes of lymph node metastasis influences. Results Tho- racic esophageal cancer above the mediastinum, hilar, earina was more common;cardia cancer metastasis was mainly in the ab- dominal cavity, more common around the cardia. The results showed that the tumor infiltrated the whole layer, the length of more than 5 cm and the degree of cell differentiation was low,located in the lower thoracic tumor and the number of lymphatic sweeps, the number of tumor depth,length, degree of differentiation and other factors 〉 3 was associated with lymph node metastasis （P 〈 0.05）. The results showed that the degree of tumor differentiation,length and depth of invasion were independent influencing fac- tors of lymph node metastasis （ P 〈 0.05 ）. Conclusion The lymph node metastasis of esophageal and cardiac cancer has certain characteristics. The clinical examination of lymph node metastasis is based on the degree of tumor differentiation, the length of le- sion and the depth of infiltration.
