早期胃癌淋巴结转移与临床病理特征相关危险因素分析

摘　　要：为了研究早期胃癌患者的临床病理特征与淋巴结转移发生的相关性。本研究收集了2009年6月至2017年4月本院收治的共185例早期胃癌患者的病理资料,采用卡方检验分析早期胃癌患者发生淋巴结转移与患者的年龄、性别、肿瘤数量、肿瘤大小、组织分型、肿瘤浸润程度、分化程度以及淋巴管癌栓情况等病理特征之间可能存在关系;同时,采用Logistic回归模型对早期胃癌患者淋巴结转移的独立危险因素进行分析。结果显示,早期胃癌的淋巴结转移与患者的年龄、肿瘤大小、分型、分化程度、浸润程度以及有无淋巴管癌栓之间密切相关（p〈0.05）。Logistics回归模型分析显示,高龄、肿瘤大小〉2 cm、肿瘤分化程度低、肿瘤浸润至粘膜下层这几个因素都是早期胃癌患者淋巴结发生转移的独立危险因素（p〈0.01）。早期胃癌的淋巴结转移与患者的一系列病理特征密切相关,其中高龄、肿瘤大小〉2 cm、肿瘤分化程度低、肿瘤浸润至粘膜下层均是早期胃癌淋巴结转移的独立危险因素。The aim of the study was to investigate the correlation between clinical features and node metastasis in early gastric cancer.Clinical data of 185 patients with early gastric cancer were collected and analyzed,including gender,age,tumor size,histological type,differentiation degree,invasion depth,and lymphatic tumor cell embolus.Chi-square test was used to analyze the correlation between clinical features and lymph node metastasis in early gastric cancer.Logistic regression analysis was performed to analyze the independent risk factors of lymph node metastasis in early gastric cancer.The result showed that the lymph node metastasis in early gastric cancer was related with age,tumor size,histological type,invasion depth,and lymphatic embolus（p〈0.01）.The results of multifactor analyzed by Logistic regression analysis indicated that age over 60,tumor size over 2 cm,poorly differentiated degree,invasion to mucous layer,and lymphatic embolus were the independent risk factors of lymph node metastasis in early gastric cancer（p〈0.05）.Age over 60,tumor size over 2 cm,poorly differentiated degree,invasion to mucous layer,and lymphatic embolus were the independent risk factors of lymph node metastasis in early gastric cancer.
