摘  要:目的分析不同临床分期前列腺癌患者的差异,探讨临床分期与体重指数(BMI)的相关性。 方法回顾性分析2008年1月至2016年12月收治的363例前列腺癌患者的临床资料。年龄(74.9±8.9)岁,BMI(23.13±3.07)kg/m2。治疗前PSA 4.04~3 931.00 ng/ml。前列腺体积(57.5±36.9)ml,重量(60.4±38.7) g。PSA密度(PSAD)0.06~283.80 ng/ml2。134例有前列腺周围侵犯,113例有淋巴结转移,148例有骨转移。术后Gleason评分2~10分。比较不同临床分期患者一般资料的差异。根据患者BMI分为消瘦组、正常组、超重组、肥胖组,比较不同分组患者的临床资料,分析BMI与临床分期的相关性。 结果本研究363例的临床分期:Ⅱ期141例,Ⅲ期20例,Ⅳ期202例。患者的年龄(P=0.000)、治疗前PSA(P=0.000)、Gleason评分(P=0.000)及PSAD(P=0.008)与临床分期呈正相关,前列腺体积(P=0.268)和重量(P=0.268)与临床分期无相关性。根据BMI分为消瘦组22例、正常组205例、超重组111例、肥胖组25例,BMI与临床分期(OR=19.01,95%CI 0.05~0.25,P=0.006)、PSA(OR=14.23,95%CI 0.10~0.11,P=0.008)、Gleason评分(OR=4.77,95%CI 0.10~0.11,P=0.009)具有相关性,BMI与临床分期呈正相关。 结论不同临床分期前列腺癌患者的年龄、PSA、Gleason评分及PSAD差异明显。BMI与临床分期密切相关,BMI值越高,患者的疾病危险度越高。ObjectiveTo retrospectively analyze the different clinical stages of patients with prostate cancer, and to investigate it’s correlation with body mass index (BMI). Methods363 patients with prostate cancer were enrolled from January 2008 to December 2016. There were 141 cases of stage Ⅱ, Ⅲ in 20 cases, 202 cases of stage Ⅳ. According to the stratification of BMI(emaciation group, normal group, overweight group, obesity group), clinical data of different groups of prostate cancer patients were compared to analyze there correlation with BMI. ResultsPatient's age, pre-PSA concentration, Gleason scores and PSA density were significantly correlated with clinical stage (P〈0.05). Prostate volume and weight had no significant correlation with staging. There was a significant correlation between different strata of BMI and clinical stage (P〈0.05). ConclusionThe different strata of BMI are closely related to the clinical stage. The higher BMI, the higher risk of the prostate cancer.
