摘 要:目的:研究胃食管反流病患者肩台位置对全瓷冠边缘适合性的影响。方法:选择2014年5月至2015年5月在北京同仁医院就诊,具有全瓷单冠修复体的123例纳入研究。采用直接和间接修复体评估标准对全瓷冠的边缘适合性进行评估,并且记录肩台的位置。使用广义线性混合模型分析胃食管反流病、肩台位置对全瓷冠边缘适合性的影响。结果:123例研究对象334颗全瓷冠纳入研究,其中胃食管反流病患者有62例(164颗全瓷冠),正常人群有61例(170颗全瓷冠)。正常人群中全瓷冠的适合性好于胃食管反流病患者(a OR=2.606,P=0.009),所有研究对象中全瓷冠肩台位于龈上(a OR=3.212,P=0.001)的边缘适合性好。结论:胃食管反流病影响全瓷冠边缘适合性,龈上肩台的边缘设计全瓷冠边缘适合性好,龈上肩台设计推荐在胃食管反流病患者使用。Objective: Gastroesophageal reflux disease (GERD) may cause dental erosion. The purpose of this study was to evaluate the impact of GERD, finish line location and marginal adaptation of all ceramic crowns among patients with GERD. In addition, we evaluate the possible association of finish line location on the marginal adaptation of crowns among patients with GERD. Methods: Study participants (N=123) were recruited from the Beijing Tong Ren Hospital of Capital Medical University from May 2014 to May 2015 and had single unit ceramic crown restoration. Medical and dental histo- ries were collected by questionnaire. The distribution and degree of marginal adaptation of crowns were evaluated with clin- ical criteria for the evaluation of direct and indirect restorations. The association between marginal adaptation and covari- ates including GERD and finish line location (subgingival/supragingival) was evaluated using generalized linear mixed model. Results: Among the 123 participants enrolled in the study, 62 GERD participants had 164 crowns, and 61 healthy participants had 170 crowns. In the multilevel analysis, the marginal adaptation was more likely obtained in healthy partici- pants in comparison with participants with GERD (adjusted odds ratio [aOR]=2.606. P=0.009). In addition, we found supragingival (aOR=3.212. P=-0.001) finish line were more likely to result marginal adaptation in comparison with subgingi- val finish line. Conclusions: The marginal adaptation was more likely obtained in healthy participants in comparison with participants with GERD. Supragingival finishing line were more appropriate for patients with GERD(Beijing Tongren Hos- pital, 2014BJSZR- 10).
