摘 要:目的 :研究彩色血管能量成像 (Color power angiography,CPA)定量检测肝癌血管的病理学基础 ,评价其临床应用价值。方法 :应用 CPA术前显示 38例肝癌肿瘤血管的分布特征 ,计算肿瘤彩色血管平均密度 (Meancolor vessel density,MCVD)。术后肿瘤组织病理切片用 CD34单克隆抗体及抗 - 因子相关抗原多克隆抗体 (Antifac-tor related antigen,A- F8RA)进行免疫组织化学染色 ,计数肿瘤微血管密度 (Microvessel density,MVD) ,并结合临床分期进行统计学对比分析。结果 :38例肝癌中 CPA显示为 型 6例 (15 .79% ) ,瘤内仅见斑点状血流信号 ; 型 10例 (2 6 .31% ) ,瘤内可见条状、树枝状血流信号或瘤周弧形血管包绕伴分支伸入 ; 型 2 2例 (5 7.90 % ) ,瘤内血流呈树枝状分布且瘤周有血管包绕伴分支伸入。 38例肝癌 MCVD和 MVD分别为 (0 .14 16± 0 .0 4 73和 6 3.31±2 9.93) / 4 0 0倍视野。二者呈线性正相关 (r=0 .792 ,P<0 .0 1) ,与临床分期分别呈等级正相关 (rs=0 .714 ,P<0 .0 1,rs=0 .5 12 ,P<0 .0 1)。结论 :肝癌病灶内大量新生血管为 CPA检测肝癌血管的病理学基础。CPA术前能定量检测肝癌血管 ,评价血供 ,为临床诊断、治疗提供有效的血流信息。Objective:To study the pathologic basis and evaluate the clinic values of color power angioraphy(CPA)in quantitively detecting the vessels of hepatic carcinomas.Methods:Vessels of 38 hepatic carcinomas were performed with CPA preoperatively.The tumor blood flow characteristics were assesed and mean color vessel density(MCVD)was calculated for every tumor.Using CD34 monoclonal antibody and anti factor Ⅷ related antigen(A F8RA)polyclonal antibody,the microvessel density(MVD)was calculated with the method of immunohistochemistry after operation.The correlations were analysed among MCVD,MVD and clinical stage.Results:6(15 79%)of 38 hepatic carcinomas were displaying as CPA I with only dot like blood flow signals in the tumors;10(26 31%)cases were Ⅱ with stripe?branch shaped blood flow signals in tumor or arc shaped vessels around it and small branches extending into;22(57 90%)cases were Ⅲ,displaying as tree branch blood flow signals in intratumor and bigger surrounding vessels with branches extending into the peritumor region.The MCVD and MVD of 38 heptic carcinomas were 0 1416±0 0473 and 63 31±29 93/400×respectively.They showed significant linear correlation( r =0 792, P <0 01).They also showed significant rank correlations between the MCVD,MVD and clinical stage in hepatic carcinoma respectively(r s=0 714, P <0 01,r 2=0 512, P <0 01).Conclusions:The neoangiogenesis is the pathologic basis of CPA in detecting the vascularity of hepatic carcinomas.CPA can evaluate blood supply of hepatic carcinomas,quantitate the tumor vessels preoperatively and provide effective blood information for clinic diagnosis and treatment.
