眼外伤患儿急诊手术前右美托咪定滴鼻的镇静效果

摘　　要：目的评价眼外伤患儿急诊手术前右美托咪定滴鼻的镇静效果。方法急诊眼外伤行手术45例，年龄4～8岁，随机分为3组：DN1组，DN2组及对照组，术前右美托咪定滴鼻剂量分别为1．0μg/kg、2．0μg／kg。对照组则给予等量生理盐水滴鼻。每隔15min进行Funk镇静评分，记录镇静程度和情绪情况，以及对静脉穿刺、与父母分离和面罩诱导的接受度。结果DN1组、DN2组术前镇静均优于对照组（P〈0．05）。DN2组在术前镇静优于DN1组（P〈0．05）。DN1组、DN2组较对照组血压、心率均低（P〈0．05）。结论眼外伤患儿急诊手术前应用右美托咪定滴鼻能够起到良好的镇静效果，且不增加不良反应的发生，2μg/k右美托咪定滴鼻效果更佳。Objective To evaluate the sedative efficacy of nasal dexmedetomidine used for premedieation in ocular trauma children undergoing emergency surgery. Methods The data of 45 patients with ocular trauma, aged 4 - 8 years, were collected and they undergwent emergency surgery. All eases were randomly divided into 3 groups. Croup DN 1 received dexmedetomidine （ 1 μg/kg） intranasally, group DN 2 received dexmedetomidine（ 2 μg/kg） intranasally, and control group received equivalent saline intranasally. Level of sedation was assessed every 15 min. Drug acceptance, venepuneture, parental separation, face mask acceptance, postoperative sedation and postoperative pain scores were recorded. Results Levels of preoperative sedation in group DN1 and group DN2, were better than those in control group （ P 〈 0.05 ）. Levels of preoperative sedation in group DN2 were better than those in group DN1 （ P 〈 0.05 ）. Heart rate and blood pressure in group DN1 and group DN2 were lower than those of control group （ P 〈 0.05 ）. Conclusion The application of preoperative nasal dexmedetomidine （2μg/kg） before emergency surgery can play better sedative effect with less adverse events.
