摘 要:目的比较角膜基质内与结膜下注射康柏西普治疗角膜新血管形成(CNV)的效果。方法采取前瞻眭随机对照研究对2014年7月至2017年7月CNV40例(40眼)按照分层区组随机化原则分为观察组(康柏西普角膜基质内注射)和对照组(康柏西普结膜下注射),每组20例,观察治疗前后眼压、CNV面积和并发症。结果治疗前、治疗后1周、1个月和3个月两组眼压比较差异均无统计学意义(t=0.678,0.360,0.192,0.213;P=0.502,0.721,0.849,0.833)。CNV面积比较,治疗后1周观察组与对照组差异无统计学意义(t=0.116,P=0.909),而在治疗后1个月和3个月时观察组较对照组面积小(t=-3.346,-3.964;P=0.002,0.000),两组组内各时间点比较差异有统计学意义(观察组:F=12.982,P=0.000;对照组:F=6.378,P=0.001),但对照组治疗后3个月与治疗前比较差异无统计学意义(t=1.361,P=0.182)。所有患者均未出现角膜损伤、感染、眼内炎或全身不良反应等并发症。结论角膜基质内注射康柏西普是治疗CNV有效而安全的给药途径,较结膜下注射效果好。Objective To compare the efficacy of Conbercept between intrastromal injection and subconjunctival injection for corneal neovascularization (CNV). Methods A pro-spective randomized controlled trial, the data of 40 eyes of 40 patients with CNV were collected. All cases were divided randomly into observation group, 20 cases received intrasromal injection of Conbercept and control group, 20 cases received subconjunctival injection of Conbercept. The intraocular pressure (lOP), CNV area and complications were observed before and after treatment. Results The difference of lOP before and 1 week, 1 and 3 months after treatment was not statistically significant between the two groups ( t = 0. 678, 0. 360, 0. 192, 0. 213 ; P =0. 502, 0.721,0. 849, 0. 833). The difference in CNV area 1 week after treatment was not statistically significant between the two groups (t = 0. 116, P = 0. 909 ). The CNV area of observation group at 1 and 3 months after treatment were smaller than those of control group ( t = -3. 346, -3. 964 ; P = 0. 002, 0. 000). The CNV area was statistically significant on every follow-up time respectively (observation group: F = 12. 982, P = 0. 000 ; control group: F = 6. 378, P = 0. 001 ). However, the difference of CNV area wasn' t statistically significant between 3 months after treatment and before treatment in control group (t = 1. 361 ,P =0. 182). No corneal defect, corneal infection, entophthalmia or systemic adverse reaction occurred. Conclusion Intrasromal injection of Conbercept is an effective and safe mode for corneal neovascularization. The efficacy is better than that of subconjectival injection.
