不同Child-Pugh分级肝硬化患者血小板参数与凝血指标的变化研究

摘　　要：目的探讨不同Child-Pugh分级肝硬化患者血小板参数与凝血指标的变化特征。方法选取肝硬化患者98例,根据Child-Pugh分级标准分为A级、B级、C级3组,选取健康志愿者35例作为对照组,检测各组血小板参数及凝血指标水平。结果不同Child-Pugh分级肝硬化患者的PLT、PCT均低于对照组,PDW、MPV均高于对照组;PLT、PCT随着Child-Pugh分级水平的升高而降低,PDW、MPV随着Child-Pugh分级水平的升高而升高,上述差异均有统计学意义(P<0.05)。不同Child-Pugh分级肝硬化患者的PT、APTT、TT均比对照组延长,Fib均比对照组降低;PT、APTT、TT随着Child-Pugh分级水平的升高而延长,Fib随着Child-Pugh分级水平的升高而降低,上述差异均有统计学意义(P<0.05)。结论肝硬化患者均有血小板参数及凝血指标的异常改变,且与肝功能Child-Pugh分级密切相关。Objective To investigate the characteristics of platelet parameters and coagulation indexes in patients with different grades of Child-Pugh cirrhosis. Methods A total of 98 patients with cirrhosis were selected. According to the Child-Pugh classification standard,98 patients were divided into Grade A group,Grade B group and Grade C group,and 35 healthy volunteers were selected as control group. Platelet parameters and coagulation indexes were detected. Results The PLT and PCT in patients with different grades of Child-Pugh cirrhosis were lower than those of the control group,and the PDW and MPV levels were higher than those in the control group; PLT and PCT decreased with the increase of Child-Pugh classification level,and PDW and MPV increased with the increase of Child-Pugh classification level,with the differences statistically significant（ P 〈0. 05）. PT,APTT and TT in patients with different grades of Child-Pugh cirrhosis were longer than those in the control group,and Fib was lower than that in the control group; PT,APTT and TT increased with the increase of Child-Pugh classification level,and Fib decreased with the increase of Child-Pugh classification level,with the differences statistically significant（ P 〈0. 05）. Conclusion There are abnormal changes of platelet parameters and coagulation indexes in patients with cirrhosis,which is closely related to Child-Pugh classification.
