利多卡因胶浆在经尿道膀胱内药物灌注化疗中的应用研究

摘　　要：目的观察利多卡因胶浆在男性膀胱肿瘤患者电切术后经尿道膀胱内药物灌注化疗中的应用效果。方法收集作者科室经尿道膀胱肿瘤电切术,且术后需常规行膀胱内药物灌注化疗的男性患者59例为研究对象,按完全随机对照分组法分为试验组(n=31)和对照组(n=28)。试验组应用利多卡因胶浆作为一次性使用导尿管表面润滑剂进行膀胱内药物灌注化疗,对照组以石蜡油为表面润滑剂,观察两组患者在插入一次性使用导尿管时的疼痛程度、尿道出血、一次性插入导尿管的成功率、尿道狭窄及插入导尿管所致的不良情绪。结果试验组在插入导尿管时心理状态、一次性插入导尿管的成功率明显优于对照组(P<0.05),而疼痛程度、尿道出血及尿道狭窄的发生率明显低于对照组(P<0.05)。结论应用利多卡因胶浆作为膀胱灌注治疗中一次性使用导尿管的表面润滑剂能够降低插入导尿管所致的不良反应,增加患者在治疗过程中的舒适度和依从性,具有一定的临床应用价值。Objective To observe the effect of lidocaine gel in the intravesical instillation chemotherapy.Methods A total of 59 male patients who underwent transurethral intravesical instillation chemotherapy after the transurethral resection of bladder tumor in authors,hospital were collected and randomly divided into experimental group（n=31）and control group（n=28）.The experimental group patients were applied lidocaine gel as disposable catheters surface lubricant for intravesical instillation chemotherapy,and the control group with paraffin oil as disposable catheters surface lubricant.The pain degree of insert disposable catheters,urethrorrhagia,one-time success rate,urethral stricture rate and unhealthy mood of two groups were observed.Results The one-time success rate and unhealthy mood of experimental group were significantly better than those of control group（P〈0.05）,and the pain degree of insert disposable catheters,the urethral stricture rate of experimental group were significantly lower than those of control group（P〈0.05）.Conclusion Lidocaine gel as disposable catheters surface lubricant in the transurethral intravesical instillation chemotherapy can reduce the adverse reactions by disposable catheters,and increase the comfort and compliance of patients,which has a certain clinical value.
