颅顶部定位穿刺治疗老年性丘脑出血破入脑室的研究

摘　　要：目的探讨颅顶部体表定位丘脑穿刺血肿引流术在治疗老年性丘脑出血破入脑室病人中的临床价值。方法选取126例丘脑出血破入脑室的老年病人按入院顺序随机分为研究组与对照组,入院后均立即行双侧脑室穿刺引流术。研究组术后随即行颅顶部体表定位丘脑穿刺血肿引流术,对照组术后根据病情行立体定向丘脑血肿引流术。比较2组病人的带管时间、术后并发症及治疗效果。结果研究组带管时间为(5.0±1.2)d,显著短于对照组的带管时间(10.0±1.6)d;研究组颅内感染率(6.1%)与死亡率(15.2%)均显著低于对照组的颅内感染率(18.3%)与死亡率(35.0%);研究组的治疗有效率(46.9%)明显优于对照组(21.7%),差异均具有统计学意义(P均<0.05)。结论颅顶部体表定位丘脑穿刺血肿引流术可改善丘脑出血破入脑室的老年病人的预后。Objective To investigate the clinical value of thalamic hematoma drainage via location of parietal surface in the treatment of elderly patients with thalamus hemorrhage breaking into ventricle.Methods A total of 126 cases of elderly patients with thalamus hemorrhage breaking into ventricles were randomly divided into the study group and the control group according to admission order,and received the operation of the bilateral ventricle drainage.Then the study group was treated by thalamic hematoma drainage via location of parietal surface.The control group was treated by thalamic hematoma drainage via stereotactic technique according to the patient＇s condition.The intubation time,complications and treatment effect of two groups were compared.Results The tube time of the study group（ 5.0±1.2 d）was significantly shorter than that of the control group（ 10.0 ± 1.6 d）.The intracranial infection rate（ 6.1%） and mortality rate（ 15.2%） in the study group were lower than those（ 18.3%,35.0%） in the control group respectively.The effective rate（ 46.9%） was significantly higher in the study group than that（ 21.7%） in the control group（ P 〈0.05）.Conclusions The thalamic hematoma drainage via location of parietal surface plays a role in improving the prognosis of elderly patients with thalamus hemorrhage breaking into ventricle.
