肺癌锁骨上淋巴结转移中的临床诊断探究

摘　　要：目的分析探讨超声引导下细针穿刺联合细胞蜡块免疫组化及基因检测在诊断肺癌锁骨上淋巴结转移中的价值。方法选择巩义市人民医院2014年2月—2017年2月间收治的60例肺癌锁骨上淋巴结转移患者,在超声引导下细针穿刺,将所获标本进行细胞学涂片检查,并进行细胞蜡块免疫组化及基因检测,分析比较两种方式诊断价值。结果超声引导下细针穿刺成功取材率为69.44%,阳性检出率为46.00%,均明显低于超声引导下细针穿刺联合细胞蜡块免疫组化及基因检测的94.44%、64.71%,差异有统计学意义(P<0.05)。EGFR基因突变检测阳性突变率为68.75%。结论超声引导下细针穿刺联合细胞蜡块免疫组化及基因检测在诊断肺癌锁骨上淋巴结转移中的检出率较高。Objective To investigate the value of the detection of ultrasound guided fine needle aspiration biopsy combined with cell block immunohistochemistry and gene in the diagnosis of lung cancer with supraclavicular lymph node metastasis. Methods Specimens for cytological smears and the cell blocks of immunohistochemistry and gene analysis of 60 patients with lung cancer with supraclavicular lymph nodes in the hospital from February,2014 to February,2017 under ultrasound guided fine needle aspiration were granted and detected. The diagnosis value of two methods was compared. Results The rate of diagnostic value of ultrasound guided fine needle puncture success was 69. 44%. The positive rate was 46. 00%,which was significantly lower than that of ultrasound guided fine needle 94. 44%, and acupuncture combined with cell block immunohistochemical detection and gene 64. 71%. The difference was statistically significant（ P〈 0. 05）. EGFR gene mutation positive mutation rate was 68. 75%. Conclusion Ultrasound guided fine needle puncture combined with detection of cell blocks of immunohistochemistry and gene in the diagnosis of lung cancer with supraclavicular lymph node metastasis detection rate is high.
