复方黄杞颗粒联合溴吡斯的明治疗重症肌无力的临床疗效

摘　　要：目的：评价复方黄杞颗粒联合基础西药治疗重症肌无力脾肾亏虚证的临床疗效及其对中医症状的改善。方法：采用随机字表法将248例重症肌无力脾肾亏虚证患者分为治疗组125例和对照组123例;治疗组给予复方黄杞颗粒联合基础西药治疗,对照组为安慰剂联合基础西药治疗,疗程均为12周。采用中医症状总评分评价两组的临床疗效;通过比较两组中医症状主症积分、中医症状次证积分以及各主症得分在治疗后4周、8周、12周的差异,评价其对中医症状的改善情况。结果：治疗组总有效率为79.20%,对照组为38.21%,治疗组优于对照组（P〈0.05）。治疗后8周、12周,治疗组中医症状评分总分、主症积分、次症积分均低于对照组（P〈0.05）;治疗后8周,治疗组胞睑下垂、咀嚼与吞咽困难的症状得分低于对照组（P〈0.05）;治疗后12周,治疗组胞睑下垂、咀嚼与吞咽困难、乏力、言语蹇涩的症状得分均低于对照组（P〈0.05）。结论：复方黄杞颗粒联合溴吡斯的明治疗重症肌无力脾肾亏虚证有效,可以首先显著改善胞睑下垂、咀嚼与吞咽困难的症状,继而显著改善乏力、言语蹇涩症状。Objective： To evaluate the clinical efficacy of compound Huang Qi combined with western medicine treatment of myasthenia gravis with spleen and kidney deficiency syndrome,and the efficacy of the syndrome of traditional Chinese medicine. Methods： a randomized controlled trail was conducted,248 cases of myasthenia gravis with spleen and kidney deficiency syndrome were enrolled and divided into treatment group（ 125 cases） and control group（ 123 cases）;the treatment group was given basic treatment of Western medicine combined with compound Huang Qi,control group with placebo plus Western medicine treatment,treatment for 12 weeks. The clinical efficacy of the two groups was evaluated by the total score of TCM symptoms. By comparing the difference between the two groups of TCM symptoms,the total score of TCM symptoms and the scores of main symptoms in 4 weeks,8 weeks and 12 weeks after treatment,the improvement of TCM symptoms was evaluated. Results： the total effective rate in the treatment group was 79. 20%,the control group was 38. 21%,and the treatment group was better than the control group（ P 〈 0. 05）. After treatment,8 weeks and 12 weeks,the treatment group TCM symptoms score,main symptoms and secondary symptoms were lower than the control group（ P 〈 0. 05）; after 8 weeks of treatment,the treatment group of the eyelid ptosis,dysphagia symptoms and masticatory score lower than the control group（ P 〈 0. 05）; 12 weeks after the treatment. The treatment group of the eyelid ptosis,dysphagia,scores of symptoms and chewing fatigue,sluggish speech were lower than the control group（ P 〈 0. 05）.Conclusion： compound Huang Qi combined with bromopyzia treatment of myasthenia gravis with spleen and kidney deficiency syndrome,can significantly improve the eyelid ptosis,chewing and swallowing symptoms firstly,and significantly improve the symptoms of fatigue,sluggish speech.
