中药清胰汤对于急性坏死性胰腺炎血清白细胞介素-6和8及凝血功能的影响

摘　　要：目的：观察中药清胰汤对于急性坏死性胰腺炎患者血清白细胞介素-6和8及凝血功能的影响。方法：选择160例急性坏死性胰腺炎患者作为观察组,根据胰腺炎的严重程度将其分为重度亚组（n=48）和中度亚组（n=112）,另外选择20例健康受试者作为对照组。观察组的患者接受了包括抗生素、液体支持治疗、营养支持治疗等西医常规治疗。此外,在此基础上给予清胰汤100 m L口服,每天3次。10 d为1个疗程。在发病48 h内以及治疗第10天对患者进行血清白细胞介素-6和8及凝血功能检测。结果：发病48 h内,重度组患者血清白细胞介素-6和8明显高于中度组,而凝血酶原时间（PT）、活化部分凝血酶原时间（APTT）及纤维蛋白原（Fbg）含量明显低于中度组（P均〈0.05）。经过清胰汤治疗后,观察组血清白细胞介素-6和8明显降低,而PT、APTT及Fbg含量明显增加（P均〈0.05）。结论：清胰汤能够有效地降低急性坏死性胰腺炎患者的血清白细胞介素-6和8水平,增加PT、APTT以及Fbg水平,临床疗效好,安全性高,值得在临床上大力推广应用。Objective： To observe the effect of Qingyi Decoction on serum interleukin-6 and interleukin-8 and coagulation function in patients with acute necrotizing pancreatitis. Methods： A total of 107 patients with acute necrotizing pancreatitis were selected as the observation group. According to the severity of pancreatitis,they were divided into severe subgroup（ n = 36） and moderate subgroup（ n = 71）. another 20 healthy objects were as a control group. Patients in the observation group received routine treatment,including antibiotics,liquid support,and nutritional support. In addition,on this basis they were given Qingyi Decoction 100 m L orally,3 times a day. 10 days was for a course of treatment.Serum levels of interleukin-6 and interleukin-8 and coagulation were measured in patients within 48 h of onset and on day 10 of treatment. Results： Serum interleukin-6 and interleukin-8 were significantly higher than those in moderate group（ P 〈0. 05）. The levels of prothrombin time（ PT）,activated partial thrombin time（ APTT） and fibrinogen（ Fbg）were significantly lower than those of the moderate group（ P 〈0. 05）. After treatment with Qingyi Decoction,the levels of serum interleukin-6 and interleukin-8 were significantly decreased,while the levels of PT,APTT and Fbg were significantly increased（ P 〈0. 05）. Conclusion： Qingyi Decoction can effectively reduce the levels of serum interleukin-6 and interleukin-8 in patients with acute necrotizing pancreatitis,increase the levels of PT,APTT and Fbg,and have good clinical efficacy and high safety. It is worthy to be popularized in clinical practice.
