摘  要:目的:通过生物阻抗法(bioelectrical impedance analysis,BIA)分析探讨功能性消化不良患者营养状况。方法:选取2016年1月—2017年1月期间就诊于广东省中医院消化科的功能性消化不良(functional dyspepsia,FD)患者77例(FD组),同时选择健康人55例作为正常对照组。利用生物电阻抗原理进行人体成分的评估及比较。结果:主观整体评估(subjective global assessment,SGA)法与生物电阻抗法对营养状况的评估呈高度一致性(kappa=0.813);FD组与对照组的体质量(current weight,CW)、脂肪组织指数(fat mass index,FMI)、去脂组织指数(fat-free mass index,FFMI)及骨骼肌量(skeletal muscle mass,SMM)均有统计学意义(P〈0.05);FD中脾虚气滞证、脾胃虚寒的FFMI、SMM显著低于脾胃湿热证,差异具有统计学意义(P〈0.05)。结论:生物电阻抗法对于评价FD患者营养状况有一定的临床应用价值。Objective: To carry out the nutritional assessment of patients in functional dyspepsia( FD) by bioelectrical impedance analysis. Methods: Seventy-seven FD patients as FD group and 55 healthy cases as control group were enrolled in this retrospective study from January 2016 to January 2017 in Gastroenterology Department of Guangdong Province Traditional Chinese Medical Hospital for assessing and comparingbody composition by bioelectrical impedance analysis. Results: There were no significant differences between applicating subjective global assessment( SGA) and bioelectrical impedance analysis in assessing nutritional status( kappa = 0. 813). The differences between the FD group and the control group's current weight( CW),weight loss,fat mass index( FMI),fat-free mass index,( FFMI) and skeletal muscle mass( SMM) were significant( P 〈 0. 05). There was no significant statistic difference in body mass index( BMI),body cell mass( BCM) and phase angle( PA)( P 〉 0. 05). The FFMI and SMM were significant between cases of spleen deficiency and qi stagnation and spleen-stomach deficiency cold in FD group and that of damp-heat in the spleen-stomach in FD group( P 〈 0. 05). Conclusion: Bioelectrical impedance analysis has certain clinical value in assessing nutritional status of patients in FD.
