昏迷患儿的脑电图表现与预后的相关性分析

摘　　要：目的：分析昏迷患儿的脑电图（EEG）表现及与预后的评估价值。方法：对115例昏迷患儿进行EEG监测，根据EEG异常程度分为4组：Ⅱ级组（17例）、Ⅲ级组（66例）、Ⅳ组（16例）、Ⅴ级组（16例），对4组进行改良型格拉斯哥昏迷量表（GCS）评分，分析各组GCS评分的差异性，以及EEG异常程度与GCS评分和临床预后的相关性。结果：Ⅱ级组、Ⅲ级组、Ⅳ级组、Ⅴ级组GCS评分依次降低，差异有统计学意义。EEG分级与GCS评分呈负相关（r=-0．918，P=0．000），与预后呈正相关（r=0．818，P=0．000）。结论：昏迷患儿的昏迷程度与EEG异常程度具有相关性，根据EEG表现能判断脑功能损害的程度，这对昏迷患儿的预后评估具有重要应用价值。Objective： To analyze the manifestation and the prognostic value of EEG in the treatment of comatose children. Methods： EEG monitoring was conducted in 115 children with coma,and according to EEG abnormalities the cases were divided into 4 groups： Group Class Ⅱ（17 eases）, Group Class Ⅲ （66 cases）,Group Class Ⅳ（16 cases）and Group Class Ⅴ（16 cases）,Patients of the 4 groups re ceived examinations of modified Glasgow Coma Scale（GCS）, and then the difference of GCS scores, and the correlation among the abnormalities of EEG,GCS scores and clinical prognosis was analyzed. Results： The scores of GCS in Group Class Ⅱ ~ Ⅴ decreased successively,and the difference was statistically significant. The EEG classification was negatively correlated with the GCS scores（r=-0. 918, P = 0. 000）, and the EEG was associated with the prognosis,and the Spearman correlation coefficient was 0. 818 ,P= 0. 000. Conclusion：The coma degree of comatose children is related to the degree of EEG. abnormalities. EEG can judge the extent of their brain function damage and has important application value in the prognosis of comatose children.
