内镜下多环黏膜套切术与内镜黏膜下切除术治疗食管黏膜下肿瘤的疗效比较

摘　　要：目的比较早期食管黏膜下肿瘤患者应用内镜下多环黏膜套切术（MBM）与内镜黏膜下切除术（SMR）的治疗效果及安全性。方法选取食管黏膜下肿瘤患者40例，依据不同治疗方法分为MBM组（22例）与SMR组（18例），比较两组食管黏膜下肿瘤病灶切除成功率、手术时间、完整切除率及并发症。结果MBM组完全切除率为95.45%，SMR组完整切除率为94.44%，两组差异未见统计学意义（P〉0.05），MBM组手术时间[（31.35±4.0）min]明显短于SMR组[（47.2±4.5）min]（P〈0.05）；MBM组出血率（9.0%）明显低于SMR组（33.3%），两组比较差异均有统计学意义（P〈0.05）。两组术后食管狭窄并发症发生率比较差异未见统计学意义（P〉0.05）。术后随访3～24个月，两组均无病灶局部复发。结论食管黏膜下肿瘤患者应用MBM治疗，安全、有效、并发症少，值得临床推广应用。Objective To compare the therapeutic effect and safety of endoscopic multi-band mueosetomy （MBM） and endoscopic submucosal dissection （SMR） in patients with early esophageal submucosal tumors. Methods Forty patients with esophageal submueosal tumors were divided into MBM group （n = 22） and SMR group （n = 18 ） according to different treatment methods. The success rate of resection, operation time, complete resection rate and complications of the two groups were compared. Results The complete resection rate was 95.45% in MBM group and it was 94. 44% in SMR group, and there was no significant difference between the two groups （P 〉 0.05 ） ; the operation time in MBM group [ （31.35± 4. 0） min ] was significantly shorter than that in SMR group [ （47.2±4.5 ） min] （ P 〈 0. 05）. The bleeding rate in MBM group （9. 0% ） was significantly lower than that in SMR group （33.3% ,P 〈0. 05）. There was no significant difference in the incidence of disease between the two groups （ P 〉 0. 05 ）. The patient were followed up for 3 to 24 months and no local recurrence was found in both the two groups. Conclusions The application of MBM in patients with esophageal submucosal tumor is safe, effective and has less complication. It is worthy of clinical application.
