摘  要:目的探讨CT引导下经皮穿刺125I粒子植入术联合吉西他滨+顺铂(GC)化疗方案在早期非小细胞肺癌(NSCLC)中的应用效果。方法选取早期NSCLC患者67例,按照随机数字表法分为对照组与观察组。对照组33例实施GC化疗,观察组34例在GC化疗基础上实施CT引导下经皮穿刺125I粒子植入术治疗。观察比较两组临床疗效及治疗前后血清癌胚抗原(CEA)、胰岛素样生长因子-1(IGF-1)水平变化情况;统计两组不良反应发生情况。结果观察组肿瘤局部控制率为85.29%(29/34),临床缓解率为79.41%(27/34);均高于对照组的60.61%(20/33)、54.55%(18/33),差异有统计学意义(P〈0.05)。治疗前,两组血清CEA、IGF-1水平比较差异未见统计学意义(P〉0.05);治疗后2个周期,观察组血清CEA、IGF-1水平均低于对照组,差异有统计学意义(P〈0.05)。观察组不良反应发生率[14.71%(5/34)]与对照组[6.06%(2/33)]比较差异未见统计学意义(P〉0.05)。结论GC化疗联合CT引导下经皮穿刺125I粒子植入术可改善早期NSCLC患者血清CEA、IGF-1水平,提高肿瘤局部控制率及缓解率,且安全性较高。Objective To investigate the effects of CT guided percutaneous 125I particle implantation combined with gemcitabine plus cisplatin (GC) chemotherapy on early-stage non-small cell lung cancer (NSCLC).Methods Sixty-seven patients with early-stage NSCLC were selected and divided into control group and observation group according to the random number table method. Thirty-three patients in control group were treated by GC chemotherapy, and 34 patients in observation group were treated by CT guided percutaneous 125I particle implantation on the basis of GC chemotherapy. The clinical efficacy and the levels of serum carcinoembryonic antigen (CEA) and insulin-like growth factor-1 (IGF-1) level before and after treatment in two groups were observed and compared. The incidence of adverse reactions in the two groups was statistically analyzed.Results The local tumor control rate of observation group was 85.29% (29/34), the clinical remission rate was 79.41% (27/34), which were higher than those of control group [60.61% (20/33), 54.55% (18/33)], the differences were statistically significant (P〈0.05). There was no significant difference in levels of serum CEA, IGF-1 between the two groups before treatment (P〉0.05); but the levels of CEA, IGF-1 in observation group were lower than those in control group after 2 courses of treatment (P〈0.05). The incidence of adverse reactions of observation group [14.71% (5/34)] was not significantly different from that of control group [6.06% (2/33)], P〉0.05.Conclusions GC chemotherapy combined with CT guided percutaneous 125I particle implantation can improve the level of serum CEA and IGF-1 in patients with early-stage NSCLC, improve the local control rate and remission rate of tumor, and has high safety.
