摘 要:目的探讨超声鉴别诊断儿童肥厚性幽门狭窄(CHPS)和幽门痉挛的临床价值。方法选取2014年11月-2016年11月隆昌市人民医院收治的CHPS患儿40例作为CHPS组、41例幽门痉挛患儿作为幽门痉挛组、30例正常婴幼儿作为对照组,所有患儿均进行幽门部位的超声检查并对幽门管长度(PL)和幽门环肌厚度(MT)进行比较,评估超声鉴别诊断CHPS与幽门痉挛的效果。结果 CHPS组患儿PL、MT超声测定值明显高于幽门痉挛组和对照组,幽门痉挛组PL、MT超声测定值明显高于对照组,差异有统计学意义(P<0.05)。CHPS组患儿PL、MT超声测定值与手术中测定值比较,差异无统计学意义(P>0.05)。超声鉴别诊断CHPS和幽门痉挛的灵敏度为95.00%,特异度为93.68%,漏诊率为5.00%,误诊率为7.32%。结论超声鉴别诊断CHPS与幽门痉挛具有良好的临床价值,可对PL和MT进行测量和动态观察,准确率较高,具有方便、快捷、无创、可重复等优点。Objective To explore the clinical value of uhrasonography in ditferential diagnosis of congenital hypertrophic pyloric stenosis (CHPS) and pylorospasm. Methods Forty CHPS children treated in Longchang County People's Hospital from November 2014 to November 2016 were selected as CHPS group, 41 children with pylorospasm were selected as pylorospasm group, and 30 normal neonates were selected as control group. Ultrasonography of pylorus was conducted among all the children, the measuring values of pyloric tube length (PL) and pyloric ring muscle thickness (MT) were compared. The effect of ultrasonography in differential diagnosis of CLIPS and pylorospasm was evaluated. Results The ultrasonic measuring values of PL and MT in CLIPS group were statistically significantly higher than those in pylorospasm group and control group ( P〈0. 05 ) . The ultrasonic measuring values of PL and MT in pylorospasm group were statistically significantly higher than those in control group (P〈0. 05 ) . In CHPS group, there was no statistically significant difference in the measuring values of PL and MT between ultrasonography and surgery (P〉0. 05) . The sensitivity, specificity, missed diagnosis rate, and misdiagnosis rate of ultrasonography in differential diagnosis of CHPS and pylorospasm were 95.00% , 93.68% , 5.00% , and 7.32% , respectively. Conclusion The clinical value of ultrasonography in differential diagnosis of CHPS and pylorospasm is good, ultrasound can measure and observe PL and MT dynamically, which has the advantages of high accuracy, convenience, quickness, noninvasion, and repeatability.
