糖皮质激素治疗髓鞘发育不良脑性瘫痪患儿的临床效果

摘　　要：目的探讨糖皮质激素治疗髓鞘发育不良脑性瘫痪患儿的临床效果。方法选取髓鞘发育不良脑性瘫痪患儿64例,根据随机数表法将其分为对照组与观察组,每组32例。对照组给予脑活素治疗,观察组给予糖皮质激素治疗。比较两组患儿治疗前后日常生活能力评分(ADL评分)、并发症发生率和治疗总有效率。结果治疗前两组患儿ADL评分比较,差异未见统计学意义(P>0.05);治疗后观察组ADL评分高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率(6.26%)低于对照组(21.88%),差异有统计学意义(P<0.05);观察组治疗总有效率(96.87%)高于对照组(81.25%),差异有统计学意义(P<0.05)。结论糖皮质激素能显著改善髓鞘发育不良脑性瘫痪患儿的生活质量,降低并发症发生率。Objective To investigate the clinical effects of glucocorticoid on children with underdeveloped myelin sheath-associated cerebral palsy. Methods Sixty-four children with underdeveloped myelin sheath-associated cerebral palsy were selected and randomly divided into control group and observation group,with 32 cases in each group. The patients in the control group were given the cerebrolysin,and the patients in the observation group were treated with glucocorticoid. The ADL points,the incidence of complications and the total effective rate of the patients in the two groups were compared.Results Before treatment,there was no significant difference in the ADL points between the two groups（ P〈0. 05）; after treatment,the ADL points of the patients in the observation group were higher than those in the control group,the difference was significant（ P〈0. 05）; the incidence of complications of the patients in observation group（ 6. 26%） was lower than that in the control group（ 21. 88%）,the difference was significant（ P〈0. 05）; the total effective rate of the patients in observation group（ 96. 87%） was higher than that in the control group（ 81. 25%）,the difference was significant（ P〈0. 05）. Conclusions In the treatment of children with underdeveloped myelin sheath-associated cerebral palsy,glucocorticoidcan can improve the quality of life,and reduce the incidence of complications.
