腹腔镜全直肠系膜切除术治疗直肠癌的疗效和安全性

摘　　要：目的探讨腹腔镜全直肠系膜切除术治疗直肠癌的疗效和安全性。方法选取直肠癌患者86例，按手术方式分为对照组和观察组，每组43例。对照组行开腹全直肠系膜切除术，观察组采用腹腔镜全直肠系膜切除术，比较两组患者的手术相关指标、术后情况及并发症发生率。结果两组手术部位、手术时间、麻醉时间、淋巴清扫数目、术中输血量、术后肛门排气时间及术后开始进食时间比较差异均未见统计学意义（P均〉0．05）；观察组手术切口长度、拔除引流管时间、术后下床活动时间及术后住院时间均短于对照组，差异有统计学意义（P〈0．05）；观察组并发症发生率（6．98％）低于对照组（23．26％），差异有统计学意义（P〈0．05）。结论给予直肠癌患者腹腔镜直肠系膜切除术可有效减小手术切口，缩短术后拔除引流管时间及下床活动时间，降低并发症发生率，促进患者康复。Objective To investigate the efficacy and safety of laparoscopic total mesoreetal excision （TME） on rectal cancer. Methods Eight-six patients with rectal cancer were divided into two groups by surgical procedures, with 43 cases in each group. The control group was given open TME, observation group was given laparoscopic TME. The surgical indexes, postoperative situations and incidence of complications of the two groups were compared. Results The differences of surgical site, surgery time, anesthesia duration, number of lymph node dissection, intraoperative blood transfusion volume, postoperative anus exhaust time, postoperative eating time between the two groups were not statistically significant （ P 〉 0.05 ） ; the length of incision, drainage tube removal time, postoperative leaving bed time and hospital stays in observation group were significantly shorter than those in control group （P 〈 0.05 ） ; the incidence of complications of observation group （6.98%） was significantly lower than that of control group （23.26% , P 〈0.05）. Conclusions Laparoscopic TME can effectively reduce incision, shorten postoperative drainage tube removal time and leaving bed time, decrease the incidence of complications, condue to patients＇ recovery in treatment of rectal cancer.
