微血管减压术治疗复发三叉神经痛临床疗效

摘　　要：目的探讨微血管减压术（MVD）治疗术后复发三又神经痛的临床疗效。方法回顾性分析2010年3月至2017年3月采MVD疗26例术后复发三叉神经痛患者临床资料及疗效。结果手术后疼痛消失22例（846％），偶有疼痛但不需服药者3例（11．5％），疼痛未消失但服药可控制者1例（3．8％），较术前有所缓解；术后进行7-91个月（平均31个月）的随访，患者疼痛复发2例（7．7％），患者留有永久性面部麻木1例（3．8％）。结论三叉神经痛MVD术后复发的原因多样，责任血管压迫仍然是最主要原因，再手术仍应MVD为主。Objective To evaluate the clinical effects of microvascular decompression （ MVD ） for recurrent trigeminal neuralgia after MVD. Methods The clinical data of 26 recurrent trigeminal neuralgia patients after MVD who underwent MVD from March 2010 to March 2017 were retrospectively reviewed and analyzed. Results After the MVD, 22 （ 84.6% ） patients were cured, and 3 （ 11.5% ） patients had some pain and need no medication, 1 （3.8%） patient had some pain was controlled with medication. With a mean length of follow-up of approximately 31 months, ranged from 7 months to 91 months after surgery, 2 patients （7.7%） had recurrence, and 1 （3.8%） patient had permanent facial numbness. Conclusion Many factors are responsible for the recurrence of trigeminal neuralgia after MVD and vascular compression is still the main cause. MVD should be the first choice of the second microsurgical procedure.
