直背综合征的64排CT征象分析

摘　　要：目的研究直背综合征（SBS）的64排CT征象和意义。方法随机对1569例进行64排CT胸部检查的图像回顾性观察，测量胸廓前后径、左右径及胸椎曲度弧高，计算前后径与左右径的比值明确是否符合SBS的诊断标准，并分析其CT征象。结果67例符合SBS诊断标准；67例SBSCT上均表现为胸椎生理曲度变直，胸椎曲度弧高降低，平均约（7±116）mm；胸廓前后径平均约（73，41±1567）mm，胸廓左右径平均约（223．15±19．61）mm，胸廓前后径与左右径的比值：38例在036-037之间，19例在033-0.35之间，10例〈0.32。所有病例均见右心室受压，25例可见左心房受压和（或）心脏向左侧胸腔移位。结论CT能发现X线平片不能显示的征象，可更准确地诊断SBS，并更好地从影像学的角度解释临床症状。Objective To study the 64 slice spiral CT features of straight back syndrome and significance.Methods A retrospective study of 1569 cases of 64 shce spiral CT chest examinations was performed at random, the thoracic anteroposterior diameters, left and right diameters and thoracic curvature cambers were measured.The calculation of the ratio of anteroposterior diameter to the left and right diameter was clearly consistent with the diagnostic criteria of the straight back syndrome, and the CT signs were analyzed. Results 67 patients accord with straight back syndrome diagnosis standard; thoracic vertebrae physiological curvature straightening, thoracic curvature arc height decreased, an average of abont （ 7± 1.16 ） ram; thoracic anteroposterior diameter of an average of about （ 73.41 ± 15.67 ） mm, thoracic left and right diameter of an average about （ 223.15± 19.61 ） mm. The ratio ofanteroposterior diameter to left and right diameter： 38 cases in 0.36-0.37, 19 cases in 0.33~0.35, 10 cases were below 0.32. The right ventricle compression were found in all the patients. The left atrial compression and （ or ） heart displacement to the left thoracic cavity were found in 25 cases. Conclusion CT can find the signs that the X-ray plain film can not display, more accurately diagnose the straight back syndrome, and better explain the chnical symptoms from the perspective of imaging.
