摘 要:目的:探讨正畸力作用对移动牙牙周膜代谢的影响,以及牙周膜代谢在正畸骨改建中的作用。方法:选择临床正畸拔牙病例,在拔除第一前磨牙后,分别在尖牙的牵引移动前、牵引移动3个月时、牵引移动结束3个时间段,从移动牙的压力侧、张力侧分别提取人牙周膜细胞,进行细胞原代培养,并用RT-PCR检测其护骨素(OPG)、细胞核因子-κB受体活化因子配体(RANKL)的表达变化。结果:人牙周膜细胞原代培养成功。尖牙近中侧OPG的表达以及尖牙远中侧RANKL的表达均在尖牙移动前接近阴性,尖牙移动3个月时最强,尖牙移动结束时减弱,而且3个观察时间点两两比较,差异均有统计学意义(P<0.01)。结论:OPG、RANKL因子存在于牙周膜细胞中,正畸牙齿移动时其活性增强,可能参与正畸牙移动和局部牙槽骨改建的全过程。Objective:To investigate the effect of orthodontic force on the periodontal ligament metabolism of mobile tooth and the role of periodontal ligament metabolism in the bone remodeling during orthodontics.Methods:The cases of or-thodontic tooth extraction were selected.After the removal of the first premolar,the human periodontal ligament cells were harvested from both the pressure side and tension side of the canines in three time including before the traction movement of the canine,three months of the traction movement,and after the traction movement.The expressions of osteoprotegerin(OPG) and nuclear factor-κB ligand(RANKL)were detected by RT-PCR.Results:Human periodontal ligament cells were success-fully cultured in primary culture.Similar results were observed of OPG expression in the tension side and RANKL expression in the pressure side,which was negative before the canine movement,the expression was the highest at three months of canine movement and weakened at the end of the canine movement,and the difference was significant respectively(P〈0.01).Con-clusion:OPG and RANKL exist in periodontal ligament cells.Their activities are enhanced when teeth move during orthodon-tics and they may participate in the whole process of orthodontic tooth movement and local alveolar remodeling.
